摘要
目的:探讨瑞芬太尼用于胃癌合并糖尿病患者的麻醉效果。方法:2017年1月-2019年1月收治行胃癌根治术的糖尿病患者90例,随机分为两组,各45例。试验组给予瑞芬太尼全麻;对照组给予芬太尼诱导全麻。比较两组患者手术前后空腹血糖、不同时间点血氧饱和度、心率及血压水平和VAS评分。结果:试验组手术后30 min空腹血糖水平明显低于对照组,差异有统计学意义(P<0.05)。两组T_2~T_4时间点心率及收缩压水平均明显高于手术前,差异有统计学意义(P<0.05);SPO_2水平无明显变化;试验组T_2~T_4时间点收缩压及心率水平均明显低于对照组,差异有统计学意义(P<0.05)。试验组OAA/S、RS、VAS评分均明显低于对照组,差异有统计学意义(P<0.05)。结论:瑞芬太尼对糖尿病患者合并胃癌术中的血糖维持、血流动力学变化及疼痛的耐受力方面具有显著效果。
Objective:To explore the anesthetic effect of remifentanil in patients with gastric cancer combined with diabetes mellitus.Methods:From January 2017 to January 2019,90 diabetic patients who underwent radical gastrectomy were selected and randomly divided into two groups with 45 patients in each group.The experimental group was given remifentanil general anesthesia.The control group was given fentanyl induced general anesthesia.Fasting blood glucose,oxygen saturation,heart rate and blood pressure levels and VAS scores before and after operation at different times were compared between the two groups.Results:The fasting blood glucose level 30 min after operation in the experimental group was significantly lower than the control group,with the differences were statistically significant(P<0.05).Heart rate and systolic blood pressure level were significantly higher in both groups at time points T2~T4 than before the operation,with the differences were statistically significant(P<0.05).There was no significant change in SPO2 level.The levels of systolic blood pressure and heart rate in the experimental group were significantly lower than the control group at time points T2~T4,with the differences were statistically significant(P<0.05).OAA/S,RS and VAS scores of the experimental group were significantly lower than the control group,with the differences were statistically significant(P<0.05).Conclusion:Remifentanil has a significant effect on intraoperative blood glucose maintenance,hemodynamic changes and pain tolerance for patients with diabetes mellitus combined with gastric cancer.
