胆脂瘤型中耳炎患者血清中对氧磷酯酶1和脂氢过氧化物的表达及意义 被引量:6

摘要 目的检测对氧磷酯酶1(PON-1)、脂氢过氧化物(LOOH)在胆脂瘤型中耳炎患者血清中的表达情况并分析其临床意义.方法选择确诊的140例胆脂瘤型中耳炎患者为观察组,另选择同期的健康体检者140例为对照组,采用速率法测定血清PON-1水平;采用铁-二甲苯酚橙色复合物(FOX)法测定血清中LOOH水平;采用乳胶增强免疫透射比浊法检测血清中超敏C反应蛋白(hs-CRP)水平;采用生化法检测血清低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平;采用硫代巴比妥酸法测定血清丙二醛(MDA)含量;采用黄嘌呤氧化酶法测定血清超氧化物歧化酶(SOD)含量;采用logistic多因素回归分析胆脂瘤型中耳炎发生的影响因素;采用ROC曲线分析血清PON-1、LOOH对胆脂瘤型中耳炎的诊断价值.结果观察组血清LOOH、hs-CRP、MDA及LDL-C水平显著高于对照组,血清HDL-C、PON-1、SOD水平显著低于对照组(P<0.05);Pearson法分析结果显示,胆脂瘤型中耳炎患者血清PON-1水平与hs-CRP、MDA及LOOH水平均呈负相关,与SOD水平呈正相关,血清LOOH表达水平与hs-CRP、MDA水平均呈正相关,与SOD水平呈负相关(P<0.05);logistic回归分析结果显示,血清hs-CRP、MDA及LOOH水平高均是影响胆脂瘤型中耳炎发生的独立危险因素,SOD、PON-1水平高是影响胆脂瘤型中耳炎发生的保护因素(P<0.05).结论胆脂瘤型中耳炎患者血清PON-1表达水平降低,血清LOOH表达水平升高,可能与胆脂瘤型中耳炎发生有关. Objective To assess the expression of paraoxonase-1(PON-1) and lipid hydroperoxides(LOOH) in the serum of patients with cholesteatoma tympanitis and to analyze their clinical significance. Methods A hundred and forty patients with cholesteatoma tympanitis were selected as the observation group, and 140 healthy people who underwent physical examination at the same time were selected as the control group. The level of serum PON-1 was measured by rate method. The level of LOOH in serum was determined by ferrous oxidation xylenol orange assay(FOX). The level of high sensitive C-reactive protein(hs-CRP) in serum was assessed by latex enhanced immunotransmission turbidimetry. The levels of serum low density lipoprotein cholesterol(LDL-C) and high density lipoprotein cholesterol(HDL-C) were measured by biochemical method. The content of malondialdehyde(MDA) in serum was determined by thiobarbituric acid method. The content of serum superoxide dismutase(SOD) was measured by xanthine oxidase method. Logistic regression was used to analyze the influencing factors of cholesteatoma tympanitis;and ROC curve was used to analyze the diagnostic values of serum PON-1 and LOOH in cholesteatoma tympanitis. Results The levels of serum LOOH, hs-CRP, MDA and LDL-C in the observation group were significantly higher than those in the control group;and the levels of serum HDL-C, PON-1 and SOD were significantly lower than those in the control group(P<0.05). The results of Pearson analysis showed that the level of serum PON-1 was negatively correlated with hs-CRP, MDA and LOOH levels, and positively correlated with SOD level. The level of serum LOOH was positively correlated with the levels of hs-CRP and MDA, and negatively correlated with the level of SOD(P<0.05). The results of logistic regression analysis showed that high levels of serum hs-CRP, MDA and LOOH were independent risk factors for cholesteatoma tympanitis, and high levels of SOD and PON-1 were the protective factors of cholesteatoma tympanitis(P<0.05).
