摘要
目的观察元胡止痛方联合温针灸治疗偏头痛急性发作患者临床疗效,并探讨其作用机制。方法将94例偏头痛急性发作患者随机分为两组各47例。对照组给予元胡止痛方治疗,观察组在此基础上加以温针灸治疗。两组疗程均为7 d。对比两组临床疗效、偏头痛积分、中医证候积分、5-HT、ET-1及不良反应。结果观察组总有效率为87.23%,明显高于对照组的70.21%(P<0.05)。干预后两组患者偏头痛积分与中医证候评分均明显下降,但观察组下降幅度更大(P<0.05)。治疗后两组患者5-羟色胺(5-HT)均明显升高,但观察组升高幅度更大,内皮素-1(ET-1)均明显下降,但观察组下降更为明显(P<0.05)。对照组共出现3例(6.38%)不良反应,与观察组的5例(10.64%)对比差异无统计学意义(P>0.05)。结论元胡止痛方联合温针灸可明显提高偏头痛急性发作患者疗效,降低偏头痛积分及证候评分,其作用机制可能与降低5-HT并升高ET-1水平有关。
Objective:To investigate the clinical effect of Yuanhu Zhitong Decoction combined with warm acupuncture on patients with acute migraine attacks,and to explore its mechanism of action.Methods:A total of 94 migraine patients were divided randomly into two groups,47 cases in each group.The control group was treated with Yuanhu Zhitong Decoction,and the observation group was treated with warm acupuncture on this basis.The clinical efficacy,migraine score,TCM syndrome score,5-HT,ET-1 and adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 87.23%,which was significantly higher than 70.21%of the control group(P<0.05).After the intervention,the migraine scores and TCM syndrome scores of the two groups decreased significantly,but the decline was greater in the observation group(P<0.05);the 5-HT levels were significantly increased in the two groups,but the increase in the observation group was greater,and the ET-1 was significantly decreased,but the decrease in the observation group was more significant(P<0.05).There were 3 cases(6.38%)of adverse reactions in the control group,and there was no significant difference between the 5 cases(10.64%)in the observation group(P>0.05).Conclusion:Yuanhu Zhitong Decoction combined with conventional warm acupuncture can significantly improve the efficacy of patients with acute migraine attacks,reduce migraine scores and syndrome scores,and its mechanism may be related to reducing 5-HT and increasing ET-1 levels.
