微课堂结合远红外线疗法对血液透析病人心理弹性及血管通路通畅性的影响研究 被引量:2

摘要 [目的]探讨微课堂结合远红外线疗法对血液透析病人心理弹性及血管通路通畅性的影响。[方法]选取2017年1月—2019年2月收治的120例维持性血液透析尿毒症病人作为研究对象,随机分成对照组与试验组各60例。对照组病人血液透析24 h后采用湿热敷加喜疗妥软膏按摩,试验组病人在对照组基础上于每周3次的血液透析中使用非热康谱远红外线治疗仪治疗40 min,同时建立微信肾友圈,利用微课堂形式对病人进行健康教育。比较两组病人干预前后心理弹性及内瘘情况。[结果]干预后两组病人CD-RISC量表总分及坚韧、力量、乐观评分均高于干预前(P<0.05),且试验组病人CD-RISC量表总分及坚韧、力量、乐观评分高于对照组(P<0.05);干预后试验组病人血管充盈度好、血流量充足所占比例高于对照组,穿刺部位局部并发症发生率低于对照组(P<0.05);且试验组病人内瘘血管弹性改善优于对照组(P<0.05)。[结论]血液透析时加入远红外线治疗及微课堂教育可明显改善病人的心理弹性,提高内瘘通畅性,改善内瘘血管弹性及延长内瘘使用时间。 Objective:To study the effect of micro-classroom combined with far-infrared therapy on psychological resilience and vascular access patency in patients with hemodialysis.Methods:A total of 120 patients with maintenance hemodialysis were selected and randomly divided into control group and experimental group,60 cases in each.The patients in control group were treated with hot and humid compression and Hirudoid ointment massage after 24 hours of dialysis.The patients in experimental group were treated with non-thermal spectrum therapy instrument for 40 minutes three times a week on the basis of the control group,at the same time,WeChat kidney circle was established to conduct health education for patients in the form of micro-classroom.The psychological resilience and internal fistula were compared between two groups of patients before and after intervention.Results:After intervention,the total score of the CD-RISC scale and the scores of tenacity,strength and optimism in both groups were higher than those before intervention(P<0.05),and the score in experimental group was higher than that in control group(P<0.05).The vascular filling degree of patients in experimental group was better than that in the control group,the proportion of sufficient blood flow was better than that in the control group,and the incidence of local complications in the puncture site was lower than that in the control group(P<0.05).The vascular elasticity of the fistula of patients in the experimental group was significantly improved compared with that in the control group(P<0.05).Conclusions:Intervention of far infrared therapy and micro-classroom education during hemodialysis can improve patients′psychological resilience,improve the patency of internal fistula,increase the elasticity of internal fistula vessels and extend the application time of internal fistula.
