摘要
目的探讨脑干损伤康复患者中应用基于镜像的运动想象训练及通关利窍针刺疗法的效果。方法选取2019年3月至2020年3月本院收治的脑干损伤康复患者70例,将其分为对照组和试验组。对照组33例实施通关利窍针刺疗法,试验组37例在对照组基础上实施基于镜像的运动想象训练。采用Brunnstrom分期、偏瘫上肢功能测试(FTHUE-HK)及Barthel指数(BI)分别评估两组患者上肢和手的运动功能、日常生活能力,采用蒙特利尔认知功能评估量表(MoCA)评估患者认知功能。采用洼田饮水试验评估患者吞咽功能,并对比两组各项指标。结果试验组Brunnstrom分期、FTHUE-HK分级、BI评分、吞咽功能分级显著优于对照组[(4.87±1.64)级比(3.84±1.02)级、(5.97±1.65)级比(4.23±0.64)级、(74.35±10.78)分比(67.12±9.35)分、(1.34±0.34)级比(2.64±1.03)级,均P<0.05];干预后2周、4周、8周,试验组MoCA评分分别为(18.64±3.34)分、(20.53±3.81)分、(23.78±2.03)分,显著优于对照组的(17.02±2.35)分、(18.54±3.34)分、(20.06±1.34)(均P<0.05)。结论在脑干损伤康复患者中应用基于镜像的运动想象训练联合通关利窍针刺疗法可显著改善患者上肢功能,提高其认知,使患者日常生活能力得到显著提高。
Objective To explore the effect of image-based exercise imagination training and Tongguan Liqiao acupuncture in rehabilitation of brain stem injury.Methods From March,2019 to March,2020,70 patients with brain stem injury were divided into a control group(n=33)and an experimental group(n=37).The control group were treated with Tongguan Liqiao acupuncture,and the experimental group with image-based exercise imagination training and Tongguan Liqiao acupuncture.Brunnstrom stage,FTHUE-HK,and Barthel Index(BI)were used to evaluate the movement function and daily life ability of the upper limbs and hands of the two groups,respectively;the patients were assessed with MOCA Cognitive Function.Their swallowing function was evaluated by Water Swallow Test.The indicators of the two groups were compared.Results The Brunnstrom stage,FTHUE-HK stage,BI score,and swallowing function grade were significantly better in the experimental group than in the control group[(4.87±1.64)vs.(3.84±1.02),(5.97±1.65)vs.(4.23±0.64),(74.35±10.78)vs.(67.12±9.35),and(1.34±0.34)vs.(2.64±1.03);all P<0.05].2,4,and 8 weeks after the intervention,the scores of MOCA in the experimental group were significantly higher than those in the control group[(18.64±3.34)vs.(17.02±2.35),(20.53±3.81)vs.(18.54±3.34),and(23.78±2.03)vs.(20.06±1.34);all P<0.05].Conclusion In the rehabilitation of brain stem injury patients,the application of image-based exercise imagination training and Tongguan Liqiao acupuncture can significantly improve their upper limb function,cognition,and ability of daily life.
