摘要
目的探讨彩色多普勒超声在乳腺癌早期诊断中的应用价值。方法选择89例乳腺癌患者和37例乳腺良性病变患者,均接受彩色多普勒超声检查,记录患者的搏动指数(PI)、收缩期流速/舒张期流速(S/D)及阻力指数(RI)。以病理诊断结果为金标准,分析彩色多普勒超声对乳腺癌的诊断价值。采用受试者工作特征(ROC)曲线分析PI、S/D和RI对乳腺癌的诊断价值。结果彩色多普勒超声诊断乳腺癌的灵敏度、特异度、准确度、阳性预测值和阴性预测值分别为82.02%、89.19%、84.13%、94.81%和67.35%。彩色多普勒超声诊断非浸润性乳腺癌和浸润性乳腺癌的灵敏度分别为73.33%和86.44%,差异无统计学意义(P>0.05)。乳腺癌患者的PI、S/D和RI均明显低于乳腺良性病变患者,差异均有统计学意义(P<0.01)。PI、S/D和RI诊断乳腺良恶性病变的ROC曲线下面积分别为0.672、0.876和0.649。结论彩色多普勒超声在乳腺癌的诊断中具有较好的应用价值,可为乳腺癌的诊断提供依据。
Objective To explore the application value of color Doppler ultrasound in early diagnosis of breast cancer.Method 89 cases of breast cancer and 37 cases of benign breast lesions were selected and undertaken color Doppler ultrasound.The pulsatility index(PI),systolic to diastolic velocity ratio(S/D)and resistance index(RI)were recorded.The diagnostic value of color Doppler ultrasound was analyzed with the pathological diagnosis as the golden standard,and the diagnostic value of PI,S/D and RI for breast cancer were analyzed by the receiver operating characteristic(ROC)curve.Result The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of color Doppler ultrasound in the diagnosis of breast cancer were 82.02%,89.19%,84.13%,94.81% and 67.35%,respectively.The sensitivity of color Doppler ultrasound in the diagnosis of non-invasive breast cancer and invasive breast cancer were 73.33% and 86.44%,respectively,without statistically significant difference(P>0.05).The PI,S/D and RI of breast cancer patients were significantly lower than those of patients with benign breast lesions(P<0.01).The area under ROC curve of PI,S/D and RI for differentiating benign and malignant breast lesions were 0.672,0.876 and 0.649 respectively.Conclusion Color Doppler ultrasound are worthy of application in diagnosing breast cancer and can provide the basis for the diagnosis of breast cancer.
