摘要
目的对预防高龄髋部骨折患者压疮的护理环节的质量监控及控制,以降低高龄髋部骨折患者压疮发生率。方法选取中山大学附属第一医院关节外科2018年09月至2020年07月入院的高龄髋部骨折患者108例,按照随机数字法分为对照组和实验组,对照组54例,实施常规压疮护理,实验组54例,实施环节质量控制的护理模式。观察比较两组患者对压疮预防相关健康宣教知识的掌握程度、护理满意度评分及压疮发生率,评估环节质量控制的风险评估/判断准确率、护理干预有效率、护理相关记录合格率及风险管理合格率的改变情况,组间比较采用t检验或卡方检验。结果患者对压疮预防相关健康宣教知识的掌握程度评估,对照组为(79±13)分,实验组为(89±10)分(t=-4.585,P<0.001)。护理满意度评分:对照组为(80±15)%,实验组为(89±10)%(t=-3.449,P<0.01);实验组患者无压疮发生,明显低于对照组11%的发病率(χ^2=4.412,P<0.05)。结论加强对高龄髋部骨折患者压疮预防的重点环节干预,可提高环节质量,降低发生压疮的危险因素,有效预防压疮发生。
Objective To evaluate the effect of process quality control nursing in the prevention of hospital pressure ulcer in the aged patients with hip fractures and reduce its incidence.Methods One hundred and eight aged patients with hip fracture from the First Affiliated Hospital of Sun Yat-sen University were enrolled into this study during September 2018 to July 2020.The patients were divided into the control cohort(n=54)and the observation cohort(n=54)by the randomized digital method.Subsequently,the control cohort were administrated with regular nursing while the observation cohort were administrated with process quality control nursing for preventing pressure ulcer.The effect of health education provided by nurses,the satisfaction scores for nursing and the incidence of pressure ulcer were investigated.The assessment indexes of process quality control was further studied,including the accuracy of risk assessment,the efficiency of nursing prevention,qualification of nursing records and risk management.Measurements were analyzed by two-tailed Student’s t-test or chi-square test.Results The scores of healthy education learning from nursing in control cohort was(79±13),while the scores in observation cohort was much higher(89±10)(t=-4.585,P<0.001).The patients in observation cohort were more satisfied with nursing(89±10)%than the patients in control cohort(80±15)%(t=-3.449,P<0.01).No pressure ulcer was found in the observation cohort while six cases were found in the control cohort(χ^2=4.412,P<0.05).Conclusion Strengthening process quality control nursing can reduce the risk factors for pressure ulcer in aged patients with hip fractures,which helps for the enhanced recovery of these patients.
