摘要
目的:基于卫生费用核算体系2011(SHA 2011)核算海南省经常性卫生费用更为精细化,为相关政策制定和调整提供决策依据。方法:使用SHA 2011,核算海南省2017年经常性卫生费用。结果:2017年海南省经常性卫生费用为326.97亿元,占GDP的比重为7.33%。经常性卫生费用中家庭卫生支出占比为43.01%。;从筹资维度构成看,公共筹资方案、家庭卫生支出及自愿筹资方案占比分别为53.7%、43.01%及3.28%;从机构维度构成看,流向医院、基层医疗卫生机构、预防机构、门诊机构及药品零售机构的费用分别为48.69%、5.02%、26.52%、0.56%及1.91%;从功能维度构成看,用于治疗服务、治理、卫生行政和筹资管理、预防服务的费用分别占78.76%、17.28%、2.03%。结论:准确反映医疗费用具体情况;大部分资金流向了医院;各类医疗卫生机构功能定位明确;患者家庭医疗负担重。
Objective:Based on A System of Health Accounts 2011(SHA 2011),the calculation of Hainan’s recurrent health expenditure is more refined,providing decision-making basis for the formulation and adjustment of relevant policies.Methods:SHA2011 is used to calculate the recurrent health expenditure in Hainan Province in 2017.Results:In 2017,the recurrent health expenditure in Hainan Province is 32697 million yuan,accounting for 7.33%of GDP.The proportion of household health expenditure in recurrent health expenditure is 43.01%.The proportion of public financing,family health expenditure and voluntary financing are53.7%,43.01%and 3.28%respectively from the perspective of financing dimension;from the perspective of institutional dimension,the expenditure to hospitals,primary health care institutions,preventive institutions,outpatient institutions and drug retail institutions are 48.69%,5.02%,26.52%,0.56%and 1.91%respectively;from the perspective of functional dimension,the expenditure for administration(treatment,governance and health),financing management and prevention service are 48.69%,5.02%,26.52%,0.56%and1.91%.Services,governance,health administration and financing management,and preventive services accounted for 78.76%,17.28%and 2.03%respectively.Conclusion:It accurately reflects the specific situation of medical expenses;most of the funds flow to hospitals;the functional orientation of various medical and health institutions is clear;the burden of family medical care of patients is heavy.
