摘要
目的:探讨桥本甲状腺炎(HT)患者外周血雌二醇、白细胞分化抗原195(CD195)水平与桥本甲状腺炎的相关性。方法:选择80例女性HT患者作为研究对象,依据HT患者的甲功检查结果分为正常组(39例)和甲功减退组(甲减组,41例),另选36例同期体检健康者作为对照组。采用流式细胞仪分析两组CD195阳性细胞百分比、采用电化学发光法检测血清雌二醇水平、采用彩色超声诊断仪分析甲状腺峡部厚径。结果:甲减组和正常组的雌二醇水平和CD195阳性细胞百分比均高于对照组,其中甲减组的增高更显著(P<0.01)。相关性分析显示,甲减组和正常组的雌激素水平和CD195阳性细胞百分比有正相关性(r=0.823,P<0.01),甲减的雌激素水平和CD195阳性细胞百分比分别与促甲状腺激素(TSH),甲状腺过氧化物酶抗体(TPO-Ab)及峡部厚径有正相关性,而与游离三碘甲状腺原氨酸(FT3)和游离甲状腺素(FT4)水平有负相关性[r(雌二醇)=0.838,0.810,0.817,-0.799,-0.803,均P<0.01;r(CD195)=0.824,0.809,0.782,-0.804,-0.829,均P<0.01]。正常组的雌激素水平和CD195阳性细胞百分比分别与TSH,TPO-Ab水平及峡部厚径有正相关性[r(雌二醇)=0.786,0.821,0.808,均P<0.01;r(CD195)=0.771,0.789,0.802,均P<0.01]。结论:雌二醇和CD195与HT的发生发展有关,外周血液雌二醇、CD195水平的异常变化可能诱导了HT的发生。
Objective:To investigate the relationship between the levels of leukocyte differentiation antigen 195(CD195) and estradiol in peripheral blood and Hashimoto thyroiditis(HT).Methods:80 female HT patients were selected and divided into normal group(39 cases) and hypothyroidism group(41 cases),and 36 healthy persons were selected as control group.The percentage of CD195 positive cells was analyzed by flow cytometry.The serum estradiol level was detected by electrochemiluminescence.The thickness of thyroid isthmus was analyzed by color Doppler ultrasound.Results:The estradiol level and the percentage of CD195 positive cells in hypothyroidism group and normal group were higher than those in control group,and the increase in the hypothyroidism group was more significant(P<0.01).Correlation analysis showed that there was a positive correlation between the estrogen level and the percentage of CD195 positive cells in hypothyroidism group and normal group(r=0.823,P<0.01).The estrogen level and the percentage of CD195 positive cells in hypothyroidism group were positively correlated with thyroid stimulating hormone(TSH),thyroid peroxidase antibody(TPO-Ab) and the thickness of thyroid isthmus,but were negatively correlated with triiodothyronine(FT3) and free thyroxine(FT4) levels [r(estradiol)=0.838,0.810,0.817,-0.799,-0.803,all P<0.01;r(CD195)=0.824,0.809,0.782,-0.804,-0.829,all P<0.01].The estrogen level and the percentage of CD195 positive cells in the normal group were positively correlated with TSH,TPO-Ab levels and the thickness of the isthmus [r(estradiol)=0.786,0.821,0.808,all P<0.01;r(CD195) =0.771,0.789,0.802,all P<0.01].Conclusion:Estradiol and CD195 are related to the occurrence and development of HT.The abnormal changes of the levels of estradiol and CD195 in peripheral blood may induce HT.
