摘要
目的:探究临床药师参与慢性房颤患者华法林抗凝质量管理的病例对照效果。方法:选择2017年1月-2020年5月在本院门诊和住院治疗的130例慢性房颤患者为观察对象。按照随机数表法将其分为两组。其中对照组65例均未实施临床药师参与的华法林抗凝质量管理;研究组65例均实施临床药师参与的华法林抗凝质量管理。记录并比较两组患者治疗期间的用药依从性、抗凝治疗知晓度以及抗凝效果。结果:研究组患者的用药依从性及抗凝治疗知晓度明显高于对照组,差异具有统计学意义(P<0.05)。研究组患者首次达到治疗范围时间、国际标准化比值(INR)不足1.5的时间比例以及INR超过2.5的时间比例均低于对照组,而INR在1.5~2.5范围的时间比例均高于对照组,差异具有统计学意义(P<0.05)。结论:基层医院慢性房颤患者在临床药师参与华法林抗凝质量管理后可以明显提高患者的用药依从性及抗凝治疗知晓度,改善其抗凝治疗效果,值得临床推广使用。
Objective:To explore the case-control effect of clinical pharmacists participating in the quality management of warfarin anticoagulation in patients with chronic atrial fibrillation in primary hospital.Methods:130 patients with chronic atrial fibrillation in our hospital from January 2017 to May 2020 were selected as the observation objects.They were divided into two groups according to random number table.Among them,65 cases in the control group did not implement warfarin anticoagulation quality management participated by clinical pharmacists,while 65 cases in the study group were all implemented warfarin anticoagulation quality management participated by clinical pharmacists.The medication compliance,awareness of anticoagulant therapy and anticoagulant effect were recorded and compared between the two groups.Results:The medication compliance and awareness of anticoagulant therapy in the study group were significantly higher than those in the control group(P<0.05).The time to reach the treatment range for the first time,the time proportion of INR less than 1.5 and the time proportion of INR exceeding 2.5 in the study group were lower than those in the control group,while the time proportion of INR in the range of 1.5-2.5 were higher than those of the control group at discharge(P<0.05).Conclusion:After clinical pharmacists participating in warfarin anticoagulant quality management in primary hospital patients with chronic atrial fibrillation,they can significantly improve their medication compliance and awareness of anticoagulant treatment,and improve the effect of anticoagulant treatment,which is worthy of clinical promotion.
