摘要
目的:探讨科室静脉治疗小组对危重患儿中心静脉通路维护持续质量改进的效果。方法:利用方便抽样法,选择2018年1月-2020年2月神经外科收治的26例危重患儿为观察组,2016年1月-2017年12月收治的31例危重患儿为对照组,在医院静脉治疗委员指导下成立静脉治疗小组,组建科室团队,学习静疗规范与维护操作流程,考核成绩合格后,参与科室危重患儿中心静脉维护。采用现象学研究对护士进行访谈,了解其在危重患儿使用中心静脉过程中的感受。结果:两组患儿中心静脉有效留置时间差异有统计学意义(P<0.05),两组患儿并发症发生率差异有统计学意义(P<0.05)。科室静疗护士认为留置中心静脉提高工作效率,减轻患儿痛苦;维护深静脉需要扎实的专科知识,指导临床护理实践。结论:科室建立静脉治疗小组对危重患儿中心静脉通路持续质量改进管理起到长足的提升,值得借鉴和推广。
Objective:To investigate the effect of department venous treatment group on the continuous quality improvement of the maintenance of central venous access in critically ill children.Method:The use of convenience sampling method,the choice between January 2018 and February 2020 neurosurgery critically ill children with 26 cases as the observation group,between January 2016 and December 2017 critically ill children with 31 cases as control group,in the hospital intravenous therapy committee established under the guidance of intravenous therapy group,set up department team,to study the static flow specification and maintenance operation process,qualified after examination,participate in critically ill children with central vein maintenance department.Phenomenological research was used to interview nurses to understand their feelings during the use of central veins in critically ill children.Results:There was a statistically significant difference in the indwelling time between the two groups(P<0.05).There was statistically significant difference in the complications of venous catheterization between the two groups(P<0.05).The intravenous therapy nurses in the department felt the indwelling of the central vein,which improved the work efficiency and alleviated the pain of the children;The maintenance of deep vein requires solid specialized knowledge to guide clinical nursing practice.Conclusion:The establishment of intravenous therapy group in the department has greatly promoted the continuous quality improvement management of central venous access in critically ill children,which is worthy of reference and promotion.
