摘要
目的:探讨超声定位在急诊ICU患者困难深静脉置管中的应用效果。方法:选取2022年2月—2023年6月清远市阳山县人民医院收治的具有置管困难因素的急诊ICU患者72例作为研究对象,按照随机数字表法分为对照组与观察组,各36例。对照组采用常规解剖定位法进行深静脉穿刺置管,观察组采用超声定位法行深静脉穿刺置管。比较两组一次穿刺成功率、置管操作时间、不良事件发生情况、死亡情况、置管操作满意度。结果:观察组一次穿刺成功率高于对照组,置管操作时间短于对照组,差异有统计学意义(P<0.05)。观察组不良事件总发生率低于对照组,差异有统计学意义(P=0.022)。两组死亡率比较,差异无统计学意义(P>0.05)。观察组置管操作总满意度高于对照组,差异有统计学意义(P=0.023)。结论:超声定位在急诊ICU患者困难深静脉置管中的应用效果显著,能提升一次穿刺成功率,缩短置管操作时间,降低不良事件发生风险,值得临床广泛应用和推广。
Objective:To explore the application effect of ultrasound Imaging in emergency ICU patients with difficulties of deep vein cannulation.Methods:72 cases of emergency ICU patients admitted to Yangshan County People's Hospital with the factor of difficult catheter placement were selected as the study subjects,and were divided into the control group and the observation group according to the method of randomized numerical table,each with 36 cases.In the control group,conventional anatomical localization method was used for deep vein puncture and cannulation,and in the observation group,ultrasonic localization method was adpoted for deep vein puncture and cannulation.The two groups were compared in terms of success rate of puncture,time of cannulation,adverse event,death,and satisfaction of cannulation.Results:The success rate of primary puncture in the observation group was higher than that in the control group,and the time of cannulation operation was shorter than that in the control group,with statistically significant differences(P<0.05).The total adverse event rate of the observation group was lower than that of the control group,and the difference was statistically significant(P=0.022).The mortality rate of the two groups was compared,and the difference was not statistically significant(P>0.05).The total satisfaction of tube placement operation in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.023).Conclusion:The application of ultrasound imaging in difficulties of deep vein cannulation in emergency ICU patients has significant effect,which can improve the success rate of one puncture,shorten the operation duration of cannulation and reduce the risk of adverse event,and is worthy of wide clinical application and promotion.
