摘要
目的:观察柴胡葛根汤加减治疗恙虫病的临床疗效。方法:88例急性恙虫病患者根据随机数字表法分为对照组44例和观察组44例。对照组给予多西环素,观察组在对照组治疗基础上给予柴胡葛根汤加减。比较两组患者症状消退时间、临床疗效、焦痂脱落时间影响因素及不良反应发生情况。结果:观察组患者体温恢复正常时间、头痛消失时间、焦痂脱落时间、肝功能恢复正常时间和降钙素原恢复正常时间均显著短于对照组(P<0.05)。观察组有效率(100.00%)显著高于对照组(90.91%),差异有统计学意义(P<0.05)。Cox回归分析显示,联合用药是影响患者焦痂脱落时间的最重要因素[HR=1.73,95%CI(1.26,2.41),P=0.028]。两组患者不良反应发生率比较,差异无统计学意义(9.08%vs 11.36%,P>0.05)。结论:柴胡葛根汤加减联合多西环素治疗恙虫病临床疗效显著,可缩短症状消退时间,改善患者预后。
Objective:To observe the clinical effect of Chaihu Gegen Decoction on tsutsugamushi disease.Methods:88 patients with acute tsutsugamushi disease were randomly divided into control group(44 cases)and observation group(44 cases).The control group was given doxycycline,and the observation group was given Chaihu Gegen Decoction on the basis of the control group.Symptoms subsided time,clinical efficacy,influencing factors of eschar shedding time and adverse reactions were compared between the two groups.Results:The normal time of body temperature,headache disappearance time,eschar shedding time,liver function recovery time and procalcitonin recovery time in the observation group were significantly shorter than those in the control group(P<0.05).The effective rate of the observation group(100.00%)was significantly higher than that of the control group(90.91%),the difference was statistically significant(P<0.05).Cox regression analysis showed that combination therapy was the most important factor affecting eschar shedding time[HR=1.73,95%CI(1.26-2.41),P=0.028].There was no significant difference in the incidence of adverse reactions between the two groups(9.08%vs 11.36%,P>0.05).Conclusion:Chaihu Gegen Decoction combined with doxycycline in the treatment of tsutsugamushi disease has significant clinical effect,which can shorten the time of symptoms subside and improve the prognosis of patients.
