摘要
目的:对比腹腔镜子宫腺肌病(AM)病灶切除术与超选择子宫动脉栓塞术治疗AM的效果。方法:选取2019年1月至2020年1月广西玉林市第二人民医院收治的40例AM患者作为观察对象,根据手术方法的不同分为腔镜组(20例)和栓塞组(20例),比较两组患者术后子宫体积变化、血红蛋白(HGB)水平、卵巢功能指标及临床疗效。结果:腔镜组术后子宫体积较栓塞组明显缩小(P<0.05),两组术后HGB水平比较,差异无统计学意义(P>0.05)。术后腔镜组雌二醇(E2)、血清卵泡刺激素(FSH)、黄体生成素(LH)水平显著低于栓塞组(P<0.05)。腔镜组和栓塞组痛经治疗效果比较,差异无统计学意义(P>0.05)。结论:腹腔镜下AM病灶切除术与子宫动脉栓塞术治疗AM临床均有显著疗效,但子宫动脉栓塞术更适合需要保留子宫和生育功能患者。
Objective:To compare the effects of laparoscopic adenomyosis(AM)lesion resection with super-selective uterine artery embolization for the treatment of AM.Methods:Forty patients with AM admitted to Yulin Second People’s Hospital from January 2019 to January 2020 were selected as study subjects and divided into a laparoscopic group(20 cases)and an embolization group(20 cases)according to different surgical methods,and the postoperative uterine volume changes,hemoglobin(HGB)levels,ovarian function indexes and clinical efficacy were compared between the two groups.Results:The postoperative uterine volume in the laparoscopic group was significantly smaller than that in the embolization group(P<0.05),and the difference in postoperative HGB level between the two groups was not statistically significant(P>0.05).The postoperative levels of estradiol(E2),serum follicle-stimulating hormone(FSH)and luteinizing hormone(LH)in the laparoscopic group were significantly lower than those in the embolization group(P<0.05).The difference between the laparoscopic and embolization groups in the treatment of dysmenorrhea was not statistically significant(P>0.05).Conclusion:Both laparoscopic AM lesion resection and uterine artery embolization were clinically effective in the treatment of AM,but uterine artery embolization was more suitable for patients who needed to preserve their uterus and reproductive function.
