摘要
目的观察加味苦参汤熏洗结合生物反馈治疗在肛瘘术后患者中的应用效果。方法选取2018年4月至2020年2月本院102例肛瘘术后患者为研究对象,随机将患者分为联合组(n=51)与对照组(n=51)。两组均接受术后常规干预,对照组加以生物反馈治疗,联合组加以加味苦参汤熏洗结合生物反馈治疗。比较两组疗效、术后疼痛、创面愈合情况,以及肛门功能恢复情况。结果联合组治疗后1个月的总有效率为94.1%(48/51),对照组为78.4%(40/51),差异有统计学意义(P<0.05)。联合组治疗后1周、2周VAS评分均低于对照组(均P<0.05),两组治疗后1个月VAS评分比较差异无统计学意义(P>0.05)。联合组治疗后1周、2周、1个月创面愈合率均高于对照组,创面渗液评分、肉芽形态评分均低于对照组(均P<0.05)。联合组治疗后1个月、2个月的肛门功能指诊评分、Wexner评分均低于对照组(均P<0.05)。结论在接受肛瘘术后常规干预的患者中,应用加味苦参汤熏洗结合生物反馈治疗相比单用生物反馈治疗促进创面愈合、肛门功能恢复及减轻术后疼痛的效果更好。
Objectives To investigate the effect of Jiawei Kushen decoction fumigation and bathing combined with biofeedback therapy in patients after anal fistula surgery.Methods One hundred and two patients who received anal fistula surgery at our hospital between April 2018 and February 2020 were recruited and randomly assigned to the combination group(n=51)and con⁃trol group(n=51).Both groups received a routine postoperative intervention.Patients in the control group received biofeedback therapy,and those in the combination group received Jiawei Kushen decoction fumigation and bathing combined with biofeedback therapy.Effectiveness,postoperative pain,wound healing,and the recovery of anal function were compared between the two groups.Results The overall effectiveness rate at 1 month after the intervention was 94.1%(48/51)and 78.4%(40/51)in the com⁃bination group and control group,respectively.There was significant difference between the two groups(P<0.05).Pain visual ana⁃log scales scores at 1 week and 2 weeks after the intervention were significantly lower in the combination group than in the con⁃trol group(P<0.05),but the scores at 1 month after the intervention did not differ between the two groups(P>0.05).The combi⁃nation group had a significantly higher percentage of wound healing at 1 week,2 weeks,and 1 months after the intervention,and lower scores of wound oozing and granulation tissue formation(P<0.05).Anal examination scores and Wexner scores at 1 month and 2 months after the intervention were also significantly lower in the combination group than in the control group(P<0.05).Conclusion Compared with biofeedback monotherapy,the combination of Jiawei Kushen decoction fumigation and bathing could facilitate wound healing and the recovery of anal function,and reduce postoperative pain in patients receiving routine inter⁃vention after anal fistula surgery.
