摘要
目的探讨耳后给药和鼓室给药对内耳耳蜗作用靶点间的区别和作用机制。方法根据不同给药方式分为耳后给药组、鼓室内给药组,所有小鼠注射0.1ml的亚甲蓝后2小时,处死动物经耳后进路,耳蜗中阶侧壁开孔,取耳蜗,显微镜下观察亚甲蓝在内耳的分布状况。结果耳后注射组亚甲蓝分布以耳蜗蜗顶最强,其次为中转,底转微弱,药物可能沿神经鞘膜逆行进入内耳。鼓室内注射组可见亚甲蓝优先进入耳蜗底转,顶回基底膜、血管纹蓝染,中回、低回也有轻微染色,但明显少于耳蜗底回,亚甲蓝可能通过圆窗膜渗透入内耳,通过内淋巴液传导至耳蜗内基底膜的毛细胞。结论耳后给药同步联合鼓室给药可以精确有效地治疗各种内耳疾病,药物可有效渗透到内耳各个部位,具有非侵入性、微创性特点,是一种新的内耳疾病治疗方式。
Objective To compare drug distribution in the cochlear via post-auricular or tympanic administration.Methods Mice were randomly selected to be injected with 0.1 ml of methylene blue either via post-auricular or intratympanic approach.Mice were sacrificed 2 hours after methylene blue administration and the cochlea was harvested following scala media fenestration via post-auricular approach for observation of methylene blue distribution in the inner ear under a microscope.Results In the post-auricular injection group,highest concentration of methylene blue was seen in the cochlear apex,followed by the mid and basal turns.In the intratympanic administration group,methylene blue appeared to enter the basal turn first,with the basilar membrane and stria vascularis in the apical turn stained blue and only slight blue staining in the mid turn(although less than in the basal turn).Conclusion We speculated that drugs may enter the inner ear in a retrograde fashion via nerve sheaths when injected post-auricularly,and penetrate the round window membrane to be transmitted to hair cells on the basilar membrane in lymphatic fluid when administered intra-tympanically..Simultaneous post-auricular and tympanic administration may be used to treat various inner ear diseases accurately and effectively.The drug can reach various parts of the inner ear via these minimally invasive routes,which can be a new treatment modality for inner ear diseases.
