摘要
目的通过CT单能量联合多伪影去除(multi-artifact reduction,MAR)技术的综合应用,以期有效去除颅内动脉瘤弹簧圈栓塞术后金属伪影并同时优化血管显示。方法收集本院2019年1-10月32例经导管颅内动脉瘤弹簧圈栓塞术后复查的患者,采用能谱成像(gemstone spectral imaging,GSI)联合MAR技术扫描采集图像,应用能谱单能量成像技术进行后处理,两位高年资介入医师对40~140 KeV的头颅血管后处理图像进行主观评分,间隔10个KeV能级为1个评分点,其中40 KeV和≥100 KeV图像质量较差被排除使用,在50~90 KeV区间,每间隔5 KeV重建9个能级的单能量图像,分别对图像的噪声值(standard deviation,SD)、信噪比(signal to noise ratio,SNR)、伪影指数(artifact index,AI)做客观统计学评价。结果进行组间SNR比较,其中65 KeV与70 KeV、75 KeV与80 KeV单能量能级比较,差异有统计学意义(P<0.05),其余组间比较差异无统计学意义;在60~65 KeV区间,AI指数较低、图像质量客观评价得分最高,提示该范围单能量能级图像对弹簧圈栓塞术后血管显示较佳。结论在60~65 KeV单能量区间,能谱CT加MAR扫描联合单能量技术对去除颅内动脉瘤弹簧圈栓塞术后金属伪影效果更好。
Objective To evaluate the effect of single-energy computed tomography(CT)combined with multi-artifact removal(MAR)for removing metal artifacts and optimizing blood vessel display in patients with coil embolization for intracranial aneurysms.Methods Thirty-two patients with intracranial aneurysms undergoing coil embolization under digital subtraction angiography(DSA)in our hospital between January and October in 2019 were enrolled in this study.After the surgery,the patients were scanned by gemstone spectral imaging(GSI)combined with MAR technique and post-processing with spectral monoenergy imaging technique.Two senior physicians scored the cranial vascular images acquired at a 10 KeV interval within 40 KeV to 140 KeV(the images acquired at 40 KeV and≥100 KeV were excluded for poor quality).Nine single-energy images were reconstructed at 5 KeV intervals between 50 KeV and 90 KeV,and the noise level,signal-to-noise ratio(SNR)and artifact index(AI)of the images were evaluated.Results The SNR of the images tended to decrease with the increase of the energy level and showed significant differences between the energy levels 65 KeV and 70 KeV and between 75 KeV and 80 KeV(P<0.05),but not between the other energy levels.The images acquired at 60~65 KeV had low AI indexes and the highest objective score for image quality with optimal display the blood vessels after coil embolization.Conclusion Energy spectral CT within the range of 60~65 KeV combined with MAR can effectively remove metal artifacts in patients after coil embolization for intracranial aneurysms.
