麻醉深度监护仪评估食道癌术后疼痛程度的有效性 被引量:2

The effectiveness of anaesthesia depth monitor to evaluate the degree of pain after esophageal cancer surgery

摘要 目的:探讨麻醉全深度监护仪监测疼痛指数(Painindex,PI)在食管癌术后患者疼痛评估的临床应用价值。方法:选取2019年7月-2020年1月在本院进行食管癌手术的患者112例,分别于术后不同时间点采用疼痛VAS评分和PI评估患者静息痛和咳嗽痛一致性和相关性。结果:加权Kappa检验结果显示,术后2h(κ值=0.682)、6h(κ值=0.653)、24h(κ值=0.639)VAS及PI评估VAS及PI评估静息痛感知的一致性均较高(均p<0.05),术后48hVAS及PI评估VAS及PI评估静息痛的一致性中等(κ值=0.525,P<0.05)。术后2、6、24、48h,两种方法评估咳嗽痛的一致性均较高(κ值分别为0.843、0.743、0.737、0.737,P<0.05)。Spearman相关性分析结果显示,食管癌患者VAS评分和PI值在术后不同时间点静息痛和咳嗽痛感知均存在相关性(p<0.01),静息痛和咳嗽痛在食管癌患者术后2h、6h及24h采用VAS评分和PI值评估r2均高于术后48h,说明其相关性更佳,VAS评分和PI值评估食管癌患者术后各时间点的咳嗽痛高于静脉痛,说明咳嗽痛相关性优于静脉痛。结论:疼痛VAS评分和PI评估静息痛和咳嗽痛结果具有一致性和相关性。PI可有效评估患者在各种状态下的疼痛程度,因其更直观科学,更合理指导临床镇痛工作的开展。 Objective:To investigate the clinical value of monitoring Pain Index(PI)with anesthesia full depth monitor in Pain assessment of postoperative patients with esophageal cancer.Methods:A total of 112 patients who underwent esophageal cancer surgery in our hospital from July 2019 to January 2020 were selected,and the pain VAS score and PI were used to evaluate the consistency and correlation of patients'resting pain and cough pain at different time points after surgery.Results:Weighted Kappa test showed high consistency for pain assessment at 2,6,24,and 24h after surgery(0.622,0.611,and 0.602,respectively,P<0.05),and moderate consistency for pain assessment at 48h after surgery(0.513,P<0.05).At 2,6,24,and 48h after surgery,the consistency of cough pain was high(values were 0.843,0.743,0.737,0.737,and 0.707,respectively,P<0.05).Spearman correlation analysis showed that VAS scores and PI values of resting pain and cough pain 2,6,24,and 24h after surgery were significantly correlated(P<0.01).R2 of resting pain were assessed by the two methods of 2,6 and 24h after>at 48h,indicating that the correlation between VAS score and PI value of resting pain at 2,6 and 24h was better than that at 48h after surgery.The r2 values of the two methods were significantly>at the same time 2,6,24 and 48h after the operation,indicating that the correlation between the pain VAS score and PI value of the cough pain was better than that of the rest pain at 2,6,24 and 48h after the operation.Conclusion:the results of the pain VAS score and PI evaluation of the rest pain and cough pain were consistent and correlated.PI can effectively evaluate the pain degree of patients in various states,because it is more intuitive and scientific,and more reasonable to guide the development of clinical analgesia.
