摘要
目的探讨精细化营养支持护理路径在食管癌围手术期中的营养效果。方法选取2015年7月至2015年12月间江苏省南通大学附属医院收治的43例食管癌根治术患者为观察组,给予精细化营养支持。选取2015年1月至2015年6月间同院收治的40例食管癌根治术患者为对照组,给予常规护理。比较两组患者术后7d的营养指标、术后并发症和住院时间等。结果观察组患者术后7d的白蛋白、血红蛋白和上臂三头肌肌围(MAMC)分别为(137.42±14.31)g/L、(37.35±4.85)g/L和(26.65±4.42)cm,均高于对照组的(129.54±15.21)g/L、(32.41±5.38)g/L和(23.47±4.83)cm,差异均有统计学意义(均P<0.05)。观察组患者腹胀、呕吐、胃潴留和吸入性肺炎等并发症发生率为14.0%,低于对照组的37.5%,差异有统计学意义(P<0.05)。观察组患者的住院时间和住院费用均少于对照组,差异均有统计学意义(均P<0.05)。结论精细化营养支持护理路径能够改善食管癌患者的术后营养状态,减少术后并发症发生率,缩短住院时间,促进患者术后康复。
Objective To investigate the efficacy of pathways for perioperative fine nutrition support in patients undergoing surgery for esophageal cancer. Methods Forty-three patients undergoing surgery for esophageal cancer at Affiliated Hospital of Nantong University from July 2015 to December 2015 were included in the observation group and they were given fine nutrition support nursing. Another 40 patients who were treated from January 2015 to June 2015 were included in the control group and they were given the convention nursing. Nutritional parameters,postoperative complications and hospitalization time at7 d after the operation were compared between the two groups. Results At 7 d after the operation,the serum albumin,hemoglobin and moveable arm muscular circulation( MAMC) was( 137. 42 ± 14. 31) g/L,( 37. 35 ± 4. 85) g/L and( 26. 65 ± 4. 42) cm,respectively for the observation group which was higher than( 129. 54 ± 15. 21) g/L,( 32. 41 ± 5. 38) g/L and( 23. 47 ± 4. 83) cm respectively of the control group( all P〈0. 05). The incidence of complications including abdominal distension,vomiting,gastric retention and aspiration pneumonia was 14. 0% in the observation group which was lower than 37. 5% of the control group( all P〈0. 05). The hospitalization time and costs were lower in the observation group than in the control group( all P〈0. 05). Conclusion Pathways for perioperative fine nutrition support can improve the nutritional status,reduce postoperative complications,shorten hospital stay and promote postoperative rehabilitation in patients undergoing surgery for esophageal cancer.
