摘要
目的分析食管癌围手术期患者应用精细化营养支持护理路径的效果。方法2018年1—12月在该院接受食管癌根治术的患者中随机抽取86例,入选患者经随机数字表法分为参考组(n=43)和研究组(n=43),为参考组患者提供营养支持指导、心理护理以及日常基础护理等常规护理干预,研究组患者采用精细化营养支持护理路径,比较护理前后两组患者上臂三头肌肌围、白蛋白、血红蛋白等营养指标,护理后两组患者并发症情况,对比两组患者住院时间。结果护理后参考组患者上臂三头肌肌围为(23.42±4.81)cm、白蛋白为(32.26±5.31)g/L、血红蛋白为(127.65±14.56)g/L,研究组患者上臂三头肌肌围为(26.64±4.77)cm、白蛋白为(37.45±5.28)g/L、血红蛋白为(137.37±13.98)g/L,两组上臂三头肌肌围差异有统计学意义(t=8.725,P<0.05),白蛋白水平差异有统计学意义(t=10.273,P<0.05),血红蛋白水平差异有统计学意义(t=11.392,P<0.05)。参考组出现并发症患者共计12例,术后并发症总发生率为27.91%,研究组出现并发症患者共计5例,术后并发症总发生率为11.63%,两组患者术后并发症总发生率差异有统计学意义(χ^2=12.025,P<0.05)。参考组患者住院时间为(17.8±1.5)d,研究组患者住院时间为(14.2±1.7)d,两组患者住院时间差异有统计学意义(t=8.423,P<0.05)。结论食管癌围手术期患者应用精细化营养支持护理路径有助于改善其术后营养状态,同时还有助于降低并发症发生率,加快其病情好转。
Objective To analyze the effect of using refined nutrition support nursing pathway in perioperative patients with esophageal cancer.Methods From January to December 2018,86 patients were randomly selected from the hospital for radical resection of esophageal cancer.The selected patients were randomly divided into reference group(n=43)and study group(n=43).For the reference group,patients were provided with nutritional support guidance,psychological nursing and daily basic nursing.The patients in the study group adopted a refined nutrition support nursing route to compare the upper arm triceps muscle circumference,albumin,hemoglobin,etc.Nutritional indicators,complications of the two groups of patients after treatment,compared the hospitalization time of the two groups of patients.Results After treatment,the upper arm triceps muscle circumference of the reference group was(23.42±4.81)cm,albumin was(32.26±5.31)g/L,hemoglobin was(127.65±14.56)g/L,and the study group patients had upper arm triceps.The muscle circumference was(26.64±4.77)cm,albumin was(37.45±5.28)g/L,hemoglobin was(137.37±13.98)g/L,and the difference in the upper arm triceps muscle circumference between the two groups was statistically significant(t=8.725,P<0.05),the difference in albumin level was statistically significant(t=10.273,P<0.05),and the difference in hemoglobin level was statistically significant(t=11.392,P<0.05).There were 12 patients with complications in the reference group.The total incidence of postoperative complications was 27.91%.There were 5 patients with complications in the study group.The total postoperative complications rate was 11.63%.The postoperative complications of the two groups were total.The difference in incidence was statistically significant(χ^2=12.025,P<0.05).The hospitalization time of the reference group was(17.8±1.5)d,and the hospitalization time of the study group was(14.2±1.7)d.The difference of hospitalization time between the two groups was statistically significant(t=8.423,P<0.05).Conclusion Perio
