摘要
目的探究骨髓涂片法联合血小板相关参数在血液疾病诊断中的应用价值。方法:收集本院于2018年4月至2019年9月收治的46例血液疾病病例作为实验组,其中包括11例特发性血小板减少性紫癜(ITP)、22例急性白血病(AL)、13例再生障碍性贫血(AA);并选取同期收治的非血液系统疾病的发热患者46例作为对比组。两组患者都进行骨髓涂片与血常规检查,记录两组患者血小板计数(PLT)、血小板体积分布宽度(PDW)、血小板平均体积(MPV)、血小板压积(PCT)与骨髓涂片的形态特征。结果实验组中不同疾病类型患者的MPV、PLT、PDW以及PCT和对比组相比有显著差异(P<0.05);实验组中不同疾病类型患者的PLT、PCT均低于对比组(P<0.05)。实验组不同疾病类型患者PDW均高于对比组(P<0.05);实验组中AL与AA患者的MPV低于对比组(P<0.05),实验组中ITP大于对比组(P<0.05)。实验组各疾病间,以及实验组与对比组间在增生程度、巨核细胞的数量与发育、血小板分布上均有显著差异。结论骨髓涂片法联合血小板相关参数在血液系统疾病诊断及鉴别诊断中,可使疾病的诊断准确率显著提高,减少误诊或漏诊情况。
Objective To explore the value of bone marrow smear combined with platelet-related parameters in the diagnosis of blood diseases.Methods:46 cases of blood diseases treated by our hospital from April 2018 to September 2019 were collected as experimental group,including 11 cases of idiopathic thrombocytopenic purpura(ITP),22 cases of acute leukemia(AL),13 cases of aplastic anemia(AA),and 46 cases of febrile patients treated with non-blood system diseases in the same period were selected as comparative group.Bone marrow smear and blood routine examination were performed in both groups.Platelet count(PLT)and platelet score were recorded in both groups morphological characteristics of cloth width(PDW),platelet mean volume(MPV),platelet hematocrit(PCT),and bone marrow smears.Results The MPV、PLT、PDW of patients with different disease types in the experimental group and the PCT group were significantly different compared with the contrast group(P<0.05);the PLT、PCT of patients with different disease types in the experimental group were lower than those in the contrast group(0.05).The PDW of patients with different disease types in the experimental group was higher than that in the contrast group(P<0.05),the MPV between AL and AA patients in the experimental group was lower than that in the contrast group(P<0.05),and the ITP in the experimental group was larger than that in the contrast group(P<0.05).There were significant differences in the degree of proliferation,the number and development of megakaryocytes and the distribution of platelets between the diseases of the experimental group and between the experimental group and the contrast group.Conclusion Bone marrow smear combined with platelet-related parameters can improve the diagnostic accuracy and reduce misdiagnosis or missed diagnosis.
