摘要
目的比较荧光定量PCR法与酶联免疫吸附法(ELISA)检测乙型肝炎病毒(HBV)的效果。方法抽取2018年1月至2019年1月于阳泉市第一人民医院就诊中发现的HBV携带者200例,于空腹8~10 h状态下清晨采取肘静脉血,均行ELISA法、荧光定量PCR法检测,将乙型肝炎标志物结果分为7个模式,其中模式1为HBsAg HBeAg HBcAb阳性(大三阳),模式2为HBsAg HBeAb HBcAb阳性(小三阳)。比较两种方法检测结果。结果 200例检测结果中乙型肝炎血清学标志物阳性模式1(大三阳)50例,HBV-DNA阳性率为98.00%(49/50)、HBV-DNA含量为(8.13±1.01)copy/ml,高于2、3、4、5、6、7模式相应值(P<0.05);模式2(小三阳)患者60例,HBV-DNA阳性率为86.67%(52/60),高于3、4、5、6、7模式的阳性率(P<0.05)。其余模式HBV-DNA含量、HBV-DNA阳性率比较差异未见统计学意义(P>0.05)。结论相较于ELISA法,荧光定量PCR法能定量反映HBV感染、复制情况,可为早期诊断乙型肝炎、传染性质判断及监测病情归转提供重要依据。
Objective To compare the effects of quantitative fluorescence PCR and enzyme-linked immunosorbent assay(ELISA)in detecting hepatitis B virus(HBV).Methods A total of 200 cases of HBV carriers who were found in Yangquan First People’s Hospital from January 2018 to January 2019 were selected.The blood of the elbow vein was taken in the early morning under the condition of fasting for 8-10 hours,and the ELISA method and Fluorescence quantitative PCR method for detection,the results of hepatitis B markers were divided into 7 modes,among which mode 1 was HBsAg HBeAg HBcAb positive(Da San Yang),and mode 2 was HBsAg HBeAb HBcAb positive(Xiao San Yang).The detection results of the two methods were compared.Results Among the 200 test results,50 cases of hepatitis B serological marker positive belonged to mode 1(Da San Yang),the HBV-DNA positive rate was 98.00%(49/50),and the HBV-DNA content was(8.13±1.01)copy/ml,which were higher than those in mode 3,4,5,6 and 7(P<0.05).There were 60 cases with mode 2(Xiao San Yang),and the positive rate of HBV-DNA was 86.67%(52/60),which were higher than those in mode 3,4,5,6 and 7(P<0.05).There was no significant difference in HBV-DNA content and HBV-DNA positive rate among other models(P>0.05).Conclusions Compared with the ELISA method,the fluorescence quantitative PCR method can quantitatively reflect the HBV infection and replication,providing important basis for early diagnosis of hepatitis B,determination of infectious nature and monitoring of disease progression.
