摘要
目的:探讨乙肝肝硬化患者乙肝病毒(HBV)-DNA水平与Toll样受体(TLR)2表达的相关性。方法:选择2018年2月至2020年3月于我院就诊的乙肝患者320例作为研究对象,根据肝硬化情况分为观察组(合并肝硬化,n=120)与对照组(未合并肝硬化,n=200),检测两组血液中HBV-DNA含量、TLR2表达水平并进行相关性分析。结果:观察组血清HBV-DNA含量高于对照组,血清TBiL、DBiL、ALP、GGT值也显著高于对照组(P<0.05)。观察组单个核细胞的TLR2表达水平显著高于对照组(P<0.05)。Pearson分析显示HBV-DNA、总胆红素(TBIL)、丙氨酸氨基转移酶(ALP)、直接胆红素(DBIL)、谷氨酰转肽酶(GGT)水平与TLR2表达呈正相关性(r=0.762,P=0.000)。回归分析显示HBV-DNA、TBiL、DBiL、ALP、GGT水平为影响TLR2表达的主要影响因素(P<0.05)。结论:乙肝肝硬化患者伴随有HBV-DNA的大量复制与TLR2的高表达,两者存在正相关性,检测HBV-DNA、TLR2表达水平对指导乙肝肝硬化患者诊疗有重要临床价值。
Objective:To investigate the correlation between Hepatitis B virus(HBV)-DNA level and Toll-like receptor(TLR)2 expression in patients with hepatitis B cirrhosis.Methods:From February 2018 to March 2020,320 cases of patients with hepatitis B diagnosed and treated in the First Hospital of Tianshui were selected as the research object.All the cases were divided into an observation group(combined cirrhosis,n=120)and control group(not combined cirrhosis,n=200)accorded to the status of cirrhosis.The HBV-DNA content and TLR2 expression level in the blood of the two groups were detected and given correlation analysis.Results:The serum HBV-DNA content of the observation group were higher than that of the control group,and the serum TBiL,DBiL,ALP,and GGT values were also higher than that of the control group,and compared the differences were statistically significant(P<0.05).The TLR2 expression level of mononuclear cells in the observation group were significantly higher than that in the control group(P<0.05).Pearson analysis showed that the HBV-DNA,Total bilirubin(TBIL),Alanine aminotransferase(ALP),Direct bilirubin(DBIL),Glutamyl transpeptidase(GGT)were positively correlated with TLR2 expression(r=0.762,P=0.000).The regression analysis showed that the levels of HBV-DNA,TBiL,DBiL,ALP,and GGT were the main factors affected the expression of TLR2(P<0.05).Conclusion:Patients with hepatitis B cirrhosis are accompanied by massive replication of HBV-DNA and high expression of TLR2.Thereare positive correlation between the two.Detection of the expression levels of HBV-DNA and TLR2 have important clinical value for the diagnosis and treatment of patients with hepatitis B cirrhosis.
