摘要
目的:系统评价双重血浆分子吸附系统(DPMAS)治疗肝衰竭的临床疗效和安全性。方法:计算机检索中国期刊全文数据库、中国生物医学文献数据库、万方数据库、PubMed、EMbase与Cochrane Library,纳入DPMAS治疗肝衰竭的随机对照试验,检索截至日期为2019年2月。共纳入8篇文献,合计523例肝衰竭患者。采用RevMan 5.3软件进行Meta分析。结果:①试验组的血清总胆红素、谷丙转氨酶、肿瘤坏死因子-ɑ、白细胞介素-6和临床总有效率均优于对照组,差异有统计学意义(P<0.05);②试验组的凝血酶原活动度、不良反应发生率与对照组比较差异无统计学意义(P>0.05)。结论:DPMAS可以更有效地治疗肝衰竭,促进肝功能恢复,减少促炎因子,且不增加不良反应发生风险,但并不能改善凝血功能。
Objective:To estimate the clinical efficacy and safety of double plasma molecular adsorbent system in treatment of patients with hepatic failure.Methods:The published literature concerning randomized controlled trials(RCTs)of double plasma molecular adsorbent system in treatment of patients with hepatic failure were retrieved from CNKI,CBM,WanFang database,PubMed,EMbase and Cochrane Library by computer before the deadline of February 2019.Meta-analysis was conducted by RevMan 5.3 software.Results:A total of eight RCTs including 523 patients with hepatic failure were enrolled.The meta-analysis demonstrated that,a)the serum total bilirubin,alanine aminotransferase,tumor necrosis factor-ɑ,interleukin-6 and clinical total effective rate of patients in the experimental group were superior to those of control group with significant differences(P<0.05);and b)there were no significant differences in prothrombin time activity and incidence of adverse reactions between the experimental group and the control group(P>0.05).Conclusion:The double plasma molecular adsorbent system can more effectively treat the patients with hepatic failure,and promote the recovery of live function and reduce the proinflammatory factors;but it can not increase the risk of adverse reactions and improve the coagulation function of patients.
