摘要
[目的]探讨经肝动脉化疗栓塞(TACE)联合选择性门静脉栓塞(SPVE)在肝癌二期切除术中的应用效果。[方法]随机将新乡市中心医院2015年6月至2017年6月收治的60例不宜开展一期手术的中晚期原发性肝癌患者分为两组,每组30例。观察组患者采取TACE+SPVE联合二期切除术治疗,对照组患者采取SPVE联合二期切除术治疗。比较两组患者治疗预后。[结果]观察组患者治疗后3 d、7 d,血清天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆红素(TBil)检测结果均低于对照组,组间对比差异显著(P<0.05);观察组患者治疗后3周左肝体积检测结果高于对照组,右肝体积、肝切除体积/肝总体积检测结果均低于对照组,组间对比差异显著(P<0.05);观察组患者二期手术切除率高于对照组,组间对比差异显著(P<0.05);观察组患者随访1 a、2 a、3 a生存率均高于对照组,组间对比差异显著(P<0.05)。[结论]对于不宜开展一期手术的中晚期原发性肝癌患者采取TACE+SPVE治疗,能够有效控制肿瘤生长、提高二期手术切除率和远期生存率。
[Objective]To explore the application effect of transcatheter arterial chemoembolization(TACE)with selected portal vein embolization(SPVE)in staged hepatectomy of hepatocellular carcinoma.[Methods]60 cases of middle and advanced stage primary which can’t treated by the one stage hepatectomy from June 2015 to June 2017 were randomly divided into observation group(30 cases)and control group(30 cases).The control group were treated by TACE+SPVE with staged hepatectomy.The observation group were treated by SPVE with staged hepatectomy.The prognosis effect was observed.[Results]The levels of AST,ALT,TBil for the observation group after 3 d and 1 weeks’treatment were lower than the control group(P<0.05);The volume of left liver for the observation group after 3 weeks’treatment was higher than the control group(P<0.05),and the volume of right liver and specific value of hepatectomy volume/total liver volume for the observation group were lower than the control group(P<0.05);The resection rate of staged hepatectomy for the observation group was higher than the control group(P<0.05);The rate of survivors for the observation group 1,2 and 3 years follow-up were higher than the control group(P<0.05).[Conclusion]It can control tumor growth in staged hepatectomy of hepatocellular carcinoma by TACE with SPVE,and it can improve the resection rate of two stage hepatectomy and long-term survival.
