摘要
目的探讨单、双排缝合技术治疗肩袖损伤对肩峰-肱骨头间距(A-H间距)的影响。方法将40例肩袖全层撕裂损伤患者按照随机数字表法分为单排组(20例)和双排组(20例),分别采用单排缝合技术与双排缝线桥缝合技术进行缝合。用西门子64排螺旋CT对两组患者术前、术后1个月A-H间距进行扫描并测量,并对两组手术前后A-H间距进行统计学分析。结果患者均获得随访,时间3~20个月。A-H间距:单排组术前为5~10(7.60±1.54)mm,术后1个月为7~13(9.25±1.59)mm;双排组术前为5~10(7.20±1.51)mm,术后1个月为7~17(12.35±2.78)mm;术后1个月,两组与术前比较差异均有统计学意义(P<0.01),两组间比较差异有统计学意义(P<0.01)。结论单、双排缝合技术均能有效改善肩袖损伤患者术后A-H间距,但双排缝线桥缝合技术对术后A-H间距改善更显著。
Objective To investigate the effect of rotator cuff injury treated with single and double rows suture technique on acromion-humeral head distance(A-H distance).Methods The 40 cases of rotator cuff full-thickness tear injuries were divided into single row group(20 cases)and double rows group(20 cases)according to the random number table method.Single row suture technique and double rows suture-bridge suture technique were used for suture respectively.The A-H distance between the two groups was scanned and measured by Siemens 64-slice spiral CT before and one month after surgery,and the A-H distance before and after surgery was statistically analyzed.Results All patients were followed up for 3~20 months.A-H distance:the single row group was 5~10(7.60±1.54)mm before surgery and 7~13(9.25±1.59)mm at 1 month after surgery.The double rows group was 5~10(7.20±1.51)mm before surgery and 7~17(12.35±2.78)mm at 1 month after surgery.At 1 month postoperation,there were statistically significant differences in the two groups,compared with before surgery(P<0.01),and there was statistically significant difference between the two groups(P<0.01).Conclusions The single row and double rows suture technique can effectively improve the postoperative A-H distance in patients with rotator cuff injury,but the double rows suture-bridge suture technique can significantly improve the A-H distance after surgery.
