基于生理相干与自主平衡系统的生物反馈训练对经皮肾穿刺活检患者术后排尿的影响 被引量:3

摘要 目的探讨肾穿刺术前基于生理相干与自主平衡系统(self-generate physiological coherence system,SPCS)的生物反馈训练对经皮肾穿刺活检患者术后排尿的影响。方法2019年1-12月,便利抽样法选择某院拟行肾穿刺活检术的患者124名为研究对象,按时间先后将其分为对照组(n=62)和观察组(n=62)。对照组患者术前予以卧床排尿训练、呼吸训练、心理护理等;观察组患者在上述基础上实施术前SPCS生物反馈训练。对比两组患者术后排尿功能、平均尿流率以及首次排尿时间,同时比较观察组患者SPCS生物反馈训练前后心率变异相关指标(平均心率、R-R间期的标准差、R-R间期差值≥50 ms的百分数、高频功率、低频功率、低频功率与高频功率之比等)及心理调节分数(协调指数、稳定指数、评价分数等)。结果SPCS生物反馈训练后,观察组患者术后排尿功能、平均尿流率以及术后首次排尿时间均明显优于对照组,差异均有统计学意义(均P<0.05);观察组SPCS生物反馈训练前后相比较,平均心率、低频功率/高频功率、低频功率均降低(均P<0.05),R-R间期的标准差、R-R间期差值≥50 ms的百分率、高频功率均明显升高(均P<0.05);稳定指数、协调指数、评价指数均显著升高,差异均有统计学意义(均P<0.01)。结论术前基于SPCS的生物反馈训练可提高患者的心理调节能力,增强应激能力,对改善肾穿刺术后排尿困难有一定积极作用。 Objective To explore the effect of preoperative Self-generate Physiological Coherence System(SPCS)on dysuria after renal puncture.Methods One hundred and twenty-four renal biopsy patients were randomly divided into a control group(n=62)and an experimental group(n=62)by convenience sampling method from January to Decembe 2019.The control group was given bed rest urination training,breathing training,psychological nursing;while the experimental group was given preoperative SPCS apart from the above mentioned training and nursing.The urinary function,the average urine flow rate,the first urination time were compared between the two groups.The relative indexes of heart rate variability before and after SPCS biofeedback training(average heart rate,the standard deviation of R-R,percentage of R-R interval above 50 ms,high-ferquency power,low-ferquency power,and ratio of H-L ferquency power),as well as the psychological adjustment scores(coordination index,stability index,evaluation score,etc.)in the observation group were observed.Results The urinary function,average urinary flow rate and the first urination time in the experimental group were significantly better than those in the control group(all P<0.05);compared with before and after SPCS intervention,M-HRT,LF/HF,LF decreased(all P<0.05),SDNN,pNN50,HF significantly increased(all P<0.05);stability index,coordination index,evaluation index significantly increased,and the difference was statistically significant(all P<0.01).Conclusions The preoperative SPCS bio-feedback training method can improve the patient’s psychological adjustment ability and enhance stress ability,to help the patient urinate as soon as possible after surgery.
