摘要
目的探讨基于不同扩散敏感梯度因子(b)值组合的体素内不相干运动扩散加权成像(Intra-Voxel Incoherent Motion Diffusion-Weighted Imaging,IVIM-DWI)模型在腹膜后肿瘤相关参数的稳定性和可重复性研究,优化IVIM-DWI模型最佳b值组合。方法回顾性分析术前行常规MR和IVIM-DWI检查的53例腹膜后肿瘤患者影像病理资料,设置5组不同的b值组合(I组:0、10、20、50、100、150、200、500、800、1000 s/mm^2;Ⅱ组:0、100、200、300、400、500、600、700、800、900 s/mm^2;Ⅲ组:0、50、100、200、400、600、800、1000、1200、1400 s/mm^2;Ⅳ组:0、50、150、500、1000 s/mm^2;Ⅴ组:0、20、100、200、800 s/mm^2),其中21例采用Ⅰ+Ⅱ+Ⅲ组b值组合方案,32例采用Ⅰ+Ⅳ+Ⅴ组b值组合方案。测量各组的IVIM-DWI定量参数(D值、f值、D^*值及ADC值),并根据李克特(Likert)5分量表法对图像质量进行评价。结果Ⅰ、Ⅱ、Ⅳ组b值组合的IVIM-DWI相关定量参数(D值、f值、D^*值及ADC值)及图像质量评分均无统计学意义(P>0.05);而Ⅲ组的D值、D^*值和ADC值均显著低于其他四组(P<0.05),且图像质量评分最低;Ⅴ组的f值和D*值与Ⅰ、Ⅱ、Ⅳ组的f值和D^*值有统计学意义(P<0.05)。此外,Ⅰ、Ⅱ、Ⅲ组b值组合的IVIM-DWI序列扫描时间约为Ⅳ和Ⅴ组的2倍。结论Ⅰ、Ⅱ、Ⅳ组b值组合的IVIM-DWI相关定量参数的稳定性和可重复性最优,其中Ⅰ组的图像质量最佳,而Ⅳ组的IVIM-DWI采集时间最短,临床工作中,可根据患者实际情况选择合适的b值组合。
Objective To investigate the stability and repeatability ofintra-voxel incoherent motion diffusion-weighted imaging(IVIM-DWI)model based on different combinations of diffusion-sensitive gradient factor(b)values in retroperitoneal tumors,and to optimize the optimal combination of bvalues of IVIM-DWI model.Methods The imaging and pathological data of 53 patients with retroperitoneal tumor who underwent routine MRI and IVIM-DWI were analyzed retrospectively.Five groups of different combinations of b values were set(I group:0,10,20,50,100,150,200,500,800,1000 s/mm^2;Ⅱgroup:0,100,200,300,400,500,600,700,800,900 s/mm^2;Ⅲgroup:0,50,100,200,400,600,800,1000,1200,1400 s/mm^2;Ⅳgroup:0,50,150,500,1000 s/mm^2;V group:0,20,100,200,800 s/mm^2).Among them,21 cases adopted the combination of bvalues inⅠ+Ⅱ+Ⅲgroup,and 32 cases adopted the combination of b-values inⅠ+Ⅳ+Ⅴgroup.IVIM-DWI quantitative parameters(D value,f value,D*value and ADC value)of each group were measured,and the image quality was evaluated by Likert 5 subscale method.Results There were no significant differences in IVIM-DWI related quantitative parameters(D value,f value,D^*value and ADC value)and image quality score(P>0.05)inⅠ+Ⅱ+Ⅲgroup,while the D value,D^*value and ADC value of theⅢgroup were significantly lower than those of the other four groups(P<0.05),and the image quality score was the lowest.The f value and D^*value of groupⅤwere significantly different from those ofⅠ,ⅡandⅣgroups(P<0.05).In addition,the IVIM-DWI sequence scanning time of the combination of b values inⅠ,ⅡandⅢgroups was approximately 2 times as long as that inⅣandⅤgroups.Conclusion The stability and repeatability of IVIM-DWI related quantitative parameters of b value combinations inⅠ,ⅡandⅣgroups were optimal,among which the image quality ofⅠgroup was the best,while the scanning time of sequence ofⅣgroup was the shortest.In clinical work,an appropriate combination of b values canbe selected according to the actual situation of patients.
