摘要
本文分析了体素内不相干运动扩散加权成像(IVIM-DWI)技术对肝细胞癌(HCC)微循环功能的评估,及其对HCC的诊断效能。选取经病理证实的120例HCC患者作为研究组,其中,病理分级Ⅰ级58例、Ⅱ级36例、Ⅲ级26例。另选取同期健康体检者120例作为对照组。两组均行3.0T核磁共振常规序列、DWI序列及IVIM序列扫描检查,测量并比较两组灌注参数表观扩散系数(ADC)、灌注分数(f)、真实扩散系数(D)及灌注相关扩散系数(D^*)值,并通过绘制受试者工作特征曲线(ROC)分析上述各参数值对HCC的预测价值。研究组ADC、D值均低于对照组,f、D^*值均高于对照组,差异有统计学意义(P<0.05);Ⅰ级患者的ADC、D值均高于Ⅱ级、Ⅲ级患者,f、D^*值均低于Ⅱ级、Ⅲ级患者,差异有统计学意义(P<0.05);Ⅱ级患者的ADC、D值均高于Ⅲ级患者,f、D^*值均低于Ⅲ级患者,差异有统计学意义(P<0.05);ADC、f、D、D^*值预测肝癌的曲线下面积分别为0.799、0.600、0.700、0.593,其中ADC、D值对肝癌的预测效果较好(P<0.05),其最佳诊断阈值分别为2.011×10^-3 mm^2/s、2.141×10^-3 mm^2/s。结果表明,IVIM-DWI技术对肝癌微循环功能评估具有较高的应用价值,其灌注参数有助于肝癌的病理分级诊断,其ADC、D值对HCC具有较高的诊断效能。
This paper mainly analyses the value of quantitative parameters of intravoxel incoherent motion diffusion-weighted imaging(IVIM-DWI)in hepatocellular carcinoma(HCC).120 cases of HCC confirmed by pathology in our hospital were selected as the research group.Among them,there were 58 cases of grade Ⅰ,36 cases of grade Ⅱ,and 26 cases of grade Ⅲ.120 healthy persons in the same period were selected as control group.Both groups were examined by conventional sequence,DWI sequence and IVIM sequence of GE Discovery MR750w 3.0T.Two sets of quantitative parameters were measured and compared:apparent diffusion coefficient(ADC),perfusion fraction(f),true diffusion coefficient(D)and perfusion-related diffusion coefficient(D^*).The predictive value of these parameters for HCC was analyzed by drawing the ROC curve.Among them,the ADC and D values of the study group were lower than those of the control group,and the f and D^* values were higher than those of the control group(P<0.05).The ADC and D values of grade Ⅰ patients were higher than those of grade Ⅱ and Ⅲ patients,and the f and D*values of grade Ⅰ patients were lower than those of grade Ⅱ and Ⅲ patients,the difference was statistically significant(P<0.05).The ADC and D values of grade Ⅱ patients were higher than those of grade Ⅲ patients,and the f and D*values were lower than those of grade Ⅲ patients(P<0.05).The area under the curve of ADC,f,D and D^* in predicting HCC is 0.799,0.600,0.700 and 0.593 respectively.Among them,ADC and D had a better predictive effect on HCC(P<0.05),and the optimal diagnostic threshold was 2.011×10^-3 mm^2/s,2.141×10^-3 mm^2/s,respectively.The results show that,IVIM-DWI has a high value in evaluating microcirculation function of hepatocellular carcinoma.Its quantitative parameters are helpful for pathological grading diagnosis of hepatocellular carcinoma.Among them,ADC and D values have a high diagnostic efficiency.
