摘要
目的:研究基于循证的医疗设备维护管理方法,提供一个客观的、标准化的方法来支持医疗设备管理工作。方法:循证维护基于证据(故障历史),首先使用一组故障类型代码对维护工作单进行分类,通过分析医疗设备的检测、预防性维护(inspection and preventive maintenance,IPM)及维修维护(corrective maintenance,CM)事件工作单,识别出现频率异常高的故障代码;然后运用一组关键绩效指标评估医疗设备的维护性能。以手术室和ICU内监护仪和注射泵为例,通过对其2015—2018年维护性能进行监测,验证基于循证的医疗设备维护管理方法的可靠性。结果:根据循证依据采取各项有针对性的预防性维护措施,包括增加更高频率的定期维护并关注更容易出现故障的配件等可减少监护仪的故障;根据循证依据采取培训措施能降低由于不正确使用及管理引起的注射泵故障。结论:基于循证的维护可以全方位监测医疗设备的维护性能,并通过改进维护策略改善其维护有效性,进而提高临床工程人员和患者的安全。
Objective To develop an evidence-based medical device maintenance management approach to facilitate objective and standardized medical device management.Methods Based on the evidences(i.e.the failures)the evidence-based maintenance used a set of fault type codes to categorize maintenance work orders,and identified the failure codes with abnormally high occurrence frequency by analyzing the work orders on inspection and preventive maintenance(IPM)and corrective maintenance(CM)for medical equipment,then a set of key performance indicators was applied to assessing the maintenance performance of medical equipment.As an example,monitors and syringe pumps in the operating room and ICU were explored over their maintenance performance from 2015 to 2018,so as to validate the reliability of the evidence-based medical equipment maintenance management approach.Results Targeted preventive maintenance measures based on evidence,including frequent scheduled maintenance and attention to failure-prone accessories,could reduce monitor failures;training measures based on evidence could decrease syringe pump failures due to incorrect use and management.Conclusion Evidence-based maintenance allows for comprehensive monitoring of the maintenance performance of medical equipment and the modification of maintenance strategies to improve the effectiveness of maintenance performance,thereby increasing the safety of clinical engineers and patients.[Chinese Medical Equipment Journal,2020,41(10):65-69,73]
