基于感染风险统计模型的微创手术设备全面质量管理体系研究 被引量:3

摘要 目的:构建基于集值统计算法的感染风险统计模型,研究提升微创手术设备全面质量管理水平的路径。方法:采用集值统计方法制定微创手术设备感染风险评估指标,从控制感染源、提升接治能力、改善控制能力、开展预警监测和制定协同控制措施方面制定感染控制管理对策。选取医院手术室在用的137台微创手术设备,按照管理方法不同分为常规组和干预组,并采用随机数表法选取同期进行手术的400例患者,分别纳入常规组和干预组,每组200例,常规组设备采用设岗专职管理,干预组采用基于集值统计算法的感染风险统计模型进行全面质量管理,对比不同管理模式下的感染风险控制效果。结果:干预组内窥镜显像系统、人工气腹系统、冲洗吸引系统、电凝电切系统、有源手术器械及辅助成像系统抽检的802例、459例、522例、595例、980例和296例抽检例数中,感染风险率分别为2.62%(21/802)、3.92%(18/459)、2.30%(12/522)、4.03%(24/595)、3.37%(33/980)和1.69%(5/296),均低于常规组,差异有统计学意义(x^(2)=16.799,x^(2)=9.157,x^(2)=13.545,x^(2)=4.075,x^(2)=5.966,x^(2)=8.923;P<0.05);干预组微创手术设备的感染来源、接治实际、控制能力、预警水平和协同控制措施指标的风险评估值分别为(75.98±4.53)、(45.90±6.09)、(72.64±3.93)、(55.98±7.92)和(50.01±5.58),均低于常规组,差异有统计学意义(t=2.574,t=6.574,t=2.645,t=3.038,t=8.539;P<0.05);医院手术室医生、护士和患者对干预组微创手术设备感染控制管理的满意度分别为(94.38±2.39)%、(95.16±2.31)%和(96.93±1.71)%,均高于对照组,差异有统计学意义(t=53897,t=6.524,t=4.647;P<0.05)。结论:基于集值统计算法的感染风险统计模型能够降低设备感染风险和不良事件发生率,提高相关工作人员和接治患者的感染风险防控意识。 Objective:To construct a statistical model of infection risk based on set-value statistical algorithm to improve the level of total quality management of minimally invasive surgical equipment.Methods:The infection risk assessment index of minimally invasive surgical equipment was developed by means of set value statistics,and the infection control management countermeasures were formulated from the aspects of controlling infection source,improving treatment capabilities,improving control capabilities,carrying out early warning and monitoring,and formulating collaborative control measures.A total of 137 minimally invasive surgical equipment used by patients admitted to the hospital were selected and divided into routine group and intervention group according to different management methods.The routine group adopted full-time post management,the intervention group adopted infection risk statistical model based on set-value statistical algorithm for total quality management.200 surgical patients treated by two groups of minimally invasive surgical equipment were selected by numerical table method,and the effects of infection risk control under different management modes were compared.Results:In the intervention group,among inspection sampled cases of the endoscopic imaging system,the artificial pneumoperitoneus system,the irrigating suction system,the electrocoagulation and electroincision system,the active surgical instrument and the auxiliary imaging system of 802 cases,459 cases,522 cases,595 cases,980 cases and 296 cases,the infection risk rates were 2.62%(21/802),3.92%(18/459),2.30%(12/522),4.03%(24/595),3.37%(33/980)and 1.69%(5/296),which were lower than those of the conventional group,the difference was statistically significant(x^(2)=16.799,x^(2)=9.157,x^(2)=13.545,x^(2)=4.075,x^(2)=5.966,x^(2)=8.923;P<0.05).The risk assessment values of minimally invasive surgical equipment of infection source,treatment practice,control capability,warning level and collaborative control measures in the intervention group we
