老年高血压患者RASS系统相关指标表达水平对嘌呤代谢物尿酸水平的影响 被引量:4

Effect of expression levels of RASS system related indicators on uric acid level in sputum metabolites in elderly patients with hypertension

摘要 目的探究老年高血压患者肾素-血管紧张素-醛固酮(RASS)系统相关指标表达水平对嘌呤代谢物尿酸(UA)水平的影响。方法前瞻性选择2017年5月至2019年5月重庆市大足区人民医院收治的123例高血压患者为高血压组,同期在我院健康体检者50例为对照组。检测并比较两组血清中血尿酸(SUA)、血管紧张素转换酶(ACE)、肾素(PRA)、醛固酮(ALD)、血管紧张素Ⅱ(AngⅡ)水平,并分析不同SUA水平的高血压患者血清PRA、ACE、AngⅡ、ALD水平。采用Pearson相关分析高血压患者血清SUA水平与血清PRA、ACE、AngⅡ、ALD水平的相关性,采用多元逐步回归分析高血压患者血清SUA水平的影响因素。结果高血压组血清SUA、PRA、ACE、AngⅡ、ALD水平显著高于对照组(P<0.05);SUA水平为351~450μmol/L的高血压患者血清PRA、ACE、AngⅡ、ALD水平显著高于SUA水平为250~350μmol/L的高血压患者(P<0.05);SUA水平为451~550μmol/L的高血压患者血清PRA、ACE、AngⅡ、ALD水平显著高于SUA水平为351~450μmol/L的高血压患者(P<0.05);高血压患者血清SUA水平与血清PRA、ACE、AngⅡ、ALD水平均呈正相关(P<0.05);以SUA为因变量,PRA、ACE、AngⅡ、ALD为自变量进行多元逐步回归分析,结果显示血清PRA、ACE、AngⅡ、ALD水平是高血压患者血清SUA水平的影响因素(P<0.05)。结论老年高血压患者血清中SUA、PRA、ACE、AngⅡ、ALD呈高水平状态,SUA水平升高与RASS系统激活导致PRA、ACE、AngⅡ、ALD水平升高密切相关,血清PRA、ACE、AngⅡ、ALD水平是老年高血压患者SUA水平升高的影响因素,这可为老年高血压患者降压和降尿酸治疗提供新的方向。 Objective To investigate the effect of expression levels of renin-angiotensin-aldosterone(RASS)system related indicators on uric acid(UA)level in sputum metabolites in elderly patients with hypertension.Methods 123 hypertension patients(hypertension group)and 50 healthy people(control group)in Chongqing Dazu District People's Hospital from May 2017 to May 2019 were enrolled.The levels of serum uric acid(SUA),angiotensin converting enzyme(ACE),plasma renin activity(PRA),aldosterone(ALD)and angiotensin II(Ang II)were detected and compared between the two groups.And the levels of serum PRA,ACE,AngⅡ,ALD in hypertensive patients with different SUA were detected.Pearson correlation analysis was used to analyze the correlation between serum SUA levels and serum levels of PRA,ACE,AngII and ALD.Multivariate stepwise regression analysis was used to analyze the influencing factors of serum SUA level in patients with hypertension.Results The serum levels of SUA,PRA,ACE,Ang II and ALD in hypertension group were significantly higher than those in control group(P<0.05).The serum levels of PRA,ACE,Ang II and ALD in hypertension patients with SUA levels of 351~450μmol/L were significantly higher than those in hypertension patients with SUA levels of 250~350μmol/L(P<0.05).The serum levels of PRA,ACE,Ang II and ALD in hypertension patients with SUA levels of 451~550μmol/L were significantly higher than those in hypertension patients with SUA levels of 351~450μmol/L(P<0.05).Serum SUA levels were positively correlated with serum PRA,ACE,Ang II and ALD levels in patients with hypertension(P<0.05).Multivariate stepwise regression analysis showed that serum PRA,ACE,Ang II and ALD levels were the influencing factors of serum SUA levels in patients with hypertension(P<0.05).Conclusion The levels of SUA,PRA,ACE,Ang II and ALD in the serum of elderly hypertensive patients are high.The elevated levels of SUA and activation of RASS system are closely related to the elevated levels of PRA,ACE,Ang II and ALD,moreover,the serum levels of P
