摘要
目的:分析三维适形放疗(3D-CRT)和紫杉醇联合顺铂(TP)同步化疗及细胞免疫对Ⅲ期非小细胞肺癌患者的效果影响。方法:收集琼海市人民医院呼吸内科124例Ⅲ期非小细胞肺癌患者作为研究对象,应用抽签法分成参照组(n=63)和试验组(n=61),参照组给予3D-CRT和TP同步化疗治疗,试验组给予3D-CRT和TP同步化疗及细胞免疫治疗。比较两组患者临床疗效、生活质量改善情况及不良反应发生情况。结果:试验组治疗总有效率为96.72%,明显高于参照组的84.13%(P<0.05)。两组治疗后生存质量评分(KPS评分)显著高于治疗前(P<0.05);试验组治疗后KPS评分高于参照组(P<0.05)。试验组不良反应发生率低于参照组,其中放射性肺炎和粒细胞降低发生率显著低于参照组,试验组发热发生率显著高于参照组(P<0.05)。结论:采用3D-CRT和TP同步化疗联合细胞免疫疗法治疗Ⅲ期非小细胞肺癌能增强临床疗效,降低放化疗相关不良反应的发生,改善患者生活质量,具有临床参考借鉴价值。
Objective:To analyze the effect of three dimensional conformal radiation therapy (3D-CRT) and Paclitaxel combined with Cisplatin (TP) synchronous chemotherapy and cellular immunity on the curative of stage Ⅲ non-small cell lung cancer patients.Method:In Qionghai People’s Hospital of Respiratory Medicine Department,124 stage Ⅲ non-small cell lung cancer patients were selected and drew lots to divide into the reference group (n=63) and the experimental group (n=61).The reference group received concurrent chemotherapy with 3D-CRT and TP,the experimental group was given 3D-CRT and TP concurrent chemotherapy and cellular immunotherapy.The clinical efficacy,improvement in quality of life and occurrence of adverse reactions were compared between the two groups.Result:The total effective rate of 96.72% in the experimental group was significantly higher than 84.13% in the reference group (P<0.05).The quality of life scores (KPS score) after treatment in both groups were significantly higher than those before treatment (P<0.05).After treatment,the KPS score in the experimental group was higher than that in the reference group (P<0.05).The incidence of the experimental group was lower than that of the reference group,the incidence of radiation pneumonia and granulocytosis in the experimental group were significantly lower than those in the reference group,the incidence of fever in the experimental group was significantly higher than that in the reference group (P<0.05).Conclusion:Using 3D-CRT and TP chemotherapy joint cellular immune therapy to treat stage Ⅲ non-small cell lung cancer can enhance clinical efficacy,reduce the occurrence of adverse reactions associated with chemoradiotherapy,improve patients’ quality of life,it has clinical reference value.
