摘要
目的探讨富血小板纤维蛋白 (platelet-rich fibrin,PRF)-骨形态发生蛋白 (bone morphogenetic protein,BMP) 联合空心加压螺钉治疗股骨颈骨折的临床疗效。方法选取 2017 年 1 月至 2019 年 3 月在我院治疗的股骨颈骨折患者 90 例,根据治疗方案分为单纯组、BMP 组和联合组,每组 30 例,其中单纯组给予空心加压螺钉治疗,BMP 组给予 BMP 联合空心加压螺钉治疗,联合组给予 PRF 和 BMP 联合空心加压螺钉治疗,观察各组骨折愈合时间、并发症发生情况、髋关节功能等。结果联合组术后髋关节功能优良率为 93.33%,优于单纯组 (P=0.005),与 BMP 组比较差异无统计学意义 (P>0.05);联合组骨折愈合时间为(6.08±0.99) 个月,明显短于单纯组和 BMP 组 (P=0.000);联合组术后 1 年日常生活能力量表 (activity of daily living scale,ADL) 评分为 (89.46±7.78) 分,明显高于单纯组和 BMP 组 (P=0.000);联合组术后 1 年股骨颈骨密度为 (0.90±0.10) g / cm^2,明显高于单纯组和 BMP 组 (P=0.000);联合组并发症发生率为 0.00%,明显低于单纯组 (P=0.008),而与 BMP 组比较差异无统计学意义 (P>0.05)。结论 PRF-BMP 联合空心加压螺钉治疗股骨颈骨折有较好的疗效,明显改善髋关节功能和生活质量且并发症较少。
Objective To investigate the clinical effect of platelet rich fibrin (PRF)-bone morphogenetic protein (BMP) combined with cannulated compression screw in the treatment of femoral neck fracture.Methods Ninety cases of femoral neck fracture,from January 2017 to March 2019,were selected.All were divided into single treatment group (n=30),BMP group (n=30),and PRF-BMP group (n=30).Cannulated compression screw was applied in the single treatment group,BMP combined with cannulated compression screw in the BMP group,and PRF-BMP in the PRF-BMP group.Healing time,complications,and hip functions of each group were observed.Results The excellent and good rate of hip functions of the PRF-BMP group (93.33%) was better than that of the single treatment group (P=0.005),and the differences between PRF-BMP group and BMP group were not statistically significant (P > 0.05).Fracture healing time of the PRF-BMP group was (6.08±0.99) months,which was significantly shorter than that of the single treatment group and BMP group (P=0.000);The Activity of Daily Living Scale (ADL) score and femoral neck bone mineral density of the PRF-BMP group were (89.46±7.78),(0.90±0.10) g / cm^2,respectively,1 year after surgery,which was significantly higher than that of the single treatment group and BMP group (P=0.000);The complication rate of PRF-BMP group was 0.00%,which was significantly lower than that of the single treatment group (P=0.008),and differences between PRF-BMP group and BMP group were not statistically significant (P > 0.05).Conclusions PRF-BMP combined with cannulated compression screw obviously improves hip functions and quality of life with less complications.
