摘要
目的:探讨磁共振扩散峰度成像(DKI)、体素内不相干运动成像(IVIM)定量参数在鼻咽癌短期疗效评估中的应用价值。方法:收集病理确诊的鼻咽癌患者40例,于治疗前、治疗3月后、治疗6月后行磁共振检查,扫描序列包括常规MRI、DKI、IVIM。使用双指数模型测得DKI参数,包括平均扩散系数(MD)、平均扩散峰度(MK)、各向异性分数(FA);IVIM参数包括纯扩散系数(D)、伪扩散系数(D*)、灌注分数(f)。治疗6月后根据Recist 1.1标准评估疗效,将患者分为病灶无残留组、病灶残留组。结果:治疗前病灶无残留组和病灶残留组MD值、MK值、D值差异有统计学意义(P<0.05)。治疗不同时间点(治疗前、治疗3月后、治疗6月后)MD值、MK值、D值、f值两两比较差异有统计学意义(P<0.05)。使用多因素联合的受试者工作曲线(ROC)分析显示,联合MD、MK、D值预测鼻咽癌疗效曲线下面积(AUC)最大,为0.911,灵敏度和特异度分别为0.833、0.857。结论:DKI和IVIM均能有效的评估鼻咽癌短期疗效,联合运用DKI及IVIM定量参数预测鼻咽癌短期疗效更有价值。
Objective:To explore the value of diffusion kurtosis imaging(DKI)and intravoxel incoherent motion magnetic resonance imaging(IVIM)in evaluating the efficacy of nasopharyngeal carcinoma(NPC).Methods:Forty patients with pathologically diagnosed NPC were enrolled.Those patients underwent conventional MRI,DKI,and IVIM at the time points of pretreatment,3 months after treatment,and 6 months after treatment.The mean diffusion coefficient(MD),mean kurtosis coefficient(MK),fractional anisotropy(FA)measured by DKI,the diffusion coefficient(D),pseudodiffusion coefficient(D*)and perfusion fraction(f)measured by IVIM were calculated via the bi-exponential model.According to the Recist 1.1(response evaluation criteria in solid tumors),all patients were divided into the non-residual group(NRG)and residual group(RG).Results:The pre-treatment MD,MK and D showed significant differences between NRG and RG(P<0.05).There were significant differences in MD,MK,D and f between different time points(P<0.05).Based on the multi-factor combined receiver operating curve(ROC)analysis,combined MD,MK,and D to predict the efficacy of NPC showed the largest area under the curve(ACU)of 0.911,and a sensitivity and specificity of 0.833 and 0.857,respectively.Conclusion:The parameters of DKI and IVIM can evaluate the efficacy of NPC.Combined pre-treatment parameters of DKI and IVIM to predict the efficacy of NPC is more valuable than using the single one.
