摘要
目的探讨鼻内镜手术在老年慢性鼻窦炎鼻息肉中的治疗效果和安全性。方法以2017年1月—2018年3月该院收治的74例慢性鼻窦炎鼻息肉患者作为研究对象,将其中年龄≥60岁的39例患者作为实验组,年龄<60岁的35例患者作为对照组,两组均接受鼻内镜手术治疗,比较治疗效果、并发症及术后复发情况。结果实验组患者的临床有效率92.31%(36/39),对照组患者的临床有效率97.14%(34/35),对照组临床疗效略高于实验组但差异无统计学意义(P>0.05);实验组患者术后并发症率10.26%(4/39),对照组术后并发症率8.57%(3/35),组间差异无统计学意义(P>0.05);两组患者的手术时间、术中失血量及住院时间3项指标比较,差异无统计学意义(P>0.05);术后6个月、1年随访结果显示两组患者的复发率比较差异无统计学意义(P>0.05)。结论结合老年慢性鼻窦炎鼻息肉患者的实际情况做好术前评估调查,鼻内镜手术治疗可取得与其他年龄阶段患者的一致的治疗效果。
Objective To explore the therapeutic effect and safety of nasal endoscopic surgery in elderly patients with chronic sinusitis and nasal polyps.Methods A total of 74 patients with chronic sinusitis and nasal polyps admitted to the Department of Otolaryngology from January 2017 to March 2018 were selected as the research object.39 of them were≥60 years old as an experimental group,35 patients aged<60 years were used as a control group.Both groups received nasal endoscopic surgery to compare treatment effects,complications,and postoperative recurrence.Results the clinical effective rate of patients in the experimental group was 92.31%(36/39),and the clinical effective rate of patients in the control group was 97.14%(34/35).The clinical efficacy of the control group was slightly higher than that of the experimental group,and there was no statistically significant difference(P>0.05);the postoperative complication rate of patients in the experimental group was 10.26%(4/39),the postoperative complication rate of the control group was 8.57%(3/35),and there was no statistical significance between the groups(P>0.05);There was no statistically significant difference in the three indexes of operation time,intraoperative blood loss and hospitalization time of the patients in the group(P>0.05);the follow-up results at 6 months and 1 year after operation showed that there was no statistically significant difference in the recurrence rate of the two groups of patients(P>0.05).Conclusion Combined with the actual situation of elderly patients with chronic sinusitis and nasal polyps,a preoperative evaluation and investigation are conducted,and endoscopic sinus surgery can achieve the same therapeutic effect as patients of other ages.
