摘要
目的从肺功能及外周血巨噬细胞中Toll样受体(TLRs)水平方面探究化痰健脾方联合沙美特罗替卡松治疗哮喘慢性持续期患儿效果。方法选择2016年5月至2019年6月陕西省第二人民医院哮喘慢性持续期患儿112例,随机数字表法分为研究组(n=56)、对照组(n=56)。对照组予以沙美特罗替卡松,研究组予以化痰健脾方联合沙美特罗替卡松,均治疗3个月。比较两组疗效、不良反应与治疗前、治疗3个月后肺功能、体液免疫功能、外周血巨噬细胞内TLR4、TLR2、TLR1表达、儿童哮喘控制测试(C-ACT)评分、儿科哮喘生命质量调查问卷(PAQLQ)评分。结果研究组总有效率为(96.43%)高于对照组(73.21%);治疗3个月后,研究组肺功能指标、体液免疫功能、C-ACT、PAQLQ评分高于对照组(P<0.05);外周血巨噬细胞内TLR4水平低于对照组,TLR2、TLR1水平高于对照组(P<0.05);两组不良反应无显著差异(P>0.05)。结论化痰健脾方联合沙美特罗替卡松治疗哮喘慢性持续期患儿疗效确切,有助于调节外周血巨噬细胞中TLRs水平,改善肺功能,增强免疫功能,提高生活质量,且安全性较高。
Objective To investigate the effects of Huayu Jianpi Recipe combined with salmeterol fluticasone on the treatment of chronic persistent asthma in children from lung function and the levels of Toll-like receptors(TLRs)in peripheral blood macrophages.Methods A total of 112 children with chronic asthma in Shanxi Second People′s Hospital from May 2016 to June 2019 were selected and divided into a study group(n=56)and a control group(n=56)by random number table method.The control group was given salmeterol and sedative,and the study group was treated with Huayu Jianpi Recipe combined with salmeterol,and all were treated for 3 months.The efficacy,adverse reactions,and pulmonary function,humoral immune function,TLR4,TLR2,and TLR1 expression in peripheral blood macrophages,lung function,children′s asthma control test(C-ACT)score,pediatric asthma quality of life questionnaire(PAQLQ)score before and 3 months after treatment were compared between the two groups.Results The total effective rate of the study group(96.43%)was higher than that of the control group(73.21%);after 3 months of treatment,the lung function index,humoral immune function,C-ACT,and PAQLQ scores in the study group were higher than those in the control group(P<0.05);the levels of TLR4 in peripheral blood macrophages were lower than those in the control group,and the levels of TLR2 and TLR1 were higher than those in the control group(P<0.05);there were no significant differences in adverse reactions between the two groups(P>0.05).Conclusion Huayu Jianpi Recipe combined with salmeterol and fluticasone for the treatment of chronic persistent asthmatic children is effective in regulating the levels of TLRs in peripheral blood macrophages,improving lung function,enhancing immune function,improving quality of life,and have higher safety.
