儿童呼吸和哮喘控制测试改良中文版在基层医院哮喘管理的应用研究 被引量:7

摘要 目的探讨儿童呼吸和哮喘控制测试(TRACK)改良中文版在基层医院5岁以下儿童支气管哮喘管理中的作用。方法选择2018年3月-2019年6月瑞金市人民医院儿科哮喘门诊就诊的初诊支气管哮喘患儿51例。于初治、治疗后第3个月、6个月及12个月时进行TRACK改良中文版评分,同时评估每阶段患儿病情严重程度、哮喘控制水平及病情分区,对比分析TRACK改良中文版测定结果与各项临床指标变化之间的相关性。比较不同管理时期患儿治疗级别与TRACK评分情况变化;同时分析不同年龄段儿童TRACK改良中文版评估哮喘控制分级水平的差异。结果严重程度为急性发作、轻度持续、中度持续及重度持续对应的TRACK分值分别为(51. 15±13. 72)分、(85. 46±5. 49)分、(80. 00±7. 75)分及(67. 68±9. 08)分,哮喘严重程度与TRACK得分情况存在相关性,哮喘严重程度越重,TRACK分值越低(P<0. 05)。控制水平为良好控制、部分控制及未控制所对应的TRACK分值分别为(87. 88±5. 04)分、(76. 12±8. 74)分及(53. 98±11. 03)分,哮喘控制水平与TRACK得分情况存在相关性,哮喘控制水平越差,TRACK分值越低(P<0. 01)。病情分区为绿区、黄区及红区所对应的TRACK得分分别为(84. 30±6. 18)分、(61. 69±11. 19)分及(46. 50±9. 73)分,不同哮喘病情分区与TRACK得分情况存在相关性,各级之间比较差别均有高度显著性(P<0. 01)。TRACK改良中文版评分指导治疗后3个月、6个月及12个月患儿病情控制及用药情况均比治疗前有明显改善,与评分两者有相关性。管理时间越长,TRACK得分越高(P<0. 05)。良好的管理使患儿用药逐渐减少,1年停药率可达55%。不同年龄不同管理时期的哮喘控制水平比较无统计学差异(P> 0. 05),提示病程演变趋势一致。结论TRACK改良中文版具有简便、快速以及有效的特点,研究所得出的分值范围在快速判断儿童哮喘临床病情分级、分区以及控制水 Objective To evaluate the clinical values of the Test for Respiratory and Asthma Control in Kids(TRACK)Modified Chi-nese Version of children under 5 years old in primary hospitals.Methods 51 infants with newly diagnosed bronchial asthma were selected from the pediatric asthma clinic of Ruijin People's Hospital from March 2018 to June 2019,At the beginning of treatment and at the 3rd,6th and 12th months after treatment,TRACK modified Chinese version score was performed.At the same time,severity degree of disease,control level of asthma and illness partition of children at each stage were evaluated.The correlation between the TRACK modified Chinese version and the changes of various clinical indicators was compared and analyzed.The changes of treatment level and TRACK scores of children in different management periods were compared.At the same time,the TRACK modified Chinese version for children of different ages was ana-lyzed to evaluate the differences in asthma control grade levels.Results The TRACK scores corresponding to acute attack,mild duration,moderate duration and severe duration were(51.15±13.72),(85.46±5.49),(80.00±7.75),and(67.68±9.08),respectively.There was a correlation between asthma severity and TRACK scores,the more severe asthma was,the lower the score of TRACK was(P<0.05).The control level was well control,partial control and no control,corresponding to the TRACK scores of(87.88±5.04),(76.12±8.74)and(53.98±11.03),respectively.There was a correlation between the control level of asthma and the score of TRACK,and the worse the control level of asthma was,the lower the TRACK score was(P<0.01).TRACK scores corresponding to green zone,yellow zone and red zone were(84.30±6.18),(61.69±11.19)and(46.50±9.73),respectively.There was a correlation between TRACK scores and different asthma condition zones,and the difference between all levels was highly significant(P<0.01).The disease control of the children at 3,6 and 12 months after the treatment were significantly improved compared with that bef
