摘要
目的:探讨伤科黄水膀胱灌注治疗女性过度活动症的临床疗效。方法:选择2018年6月—2020年1月佛山市中医院收治的女性膀胱过度活动症患者共98例,按照随机数字表法随机分为对照组和灌注组,对照组给予常规治疗,灌注组在上述的基础上加用伤科黄水膀胱灌注,疗程12周。分析治疗后的临床有效率。分析治疗前后膀胱过度活动症评分(OABSS)、尿急程度评分(IUSS)、感知膀胱症状量表(PPBC)评分、尿失禁影响程度相关问卷(IIQ-7)评分、泌尿生殖道疾病相关问卷(UDI-6)评分、膀胱过度活动症调查表(OAB-q),测定治疗前后的尿量(VV)、平均尿流率(Qavg)、最大尿流率(Qmax)、残余尿量(PVR)、急迫尿意容量(VUD)、初始尿意容量(VFD),记录24 h排尿次数、24 h尿失禁次数。结果:灌注组的临床总有效率显著高于对照组(P<0.05);两组的OABSS、IUSS、PPBC、IIQ-7、UDI-6评分、24 h排尿次数、24 h尿失禁次数较前显著降低(P<0.05),且治疗后灌注组上述指标评分显著低于对照组(P<0.05);两组治疗后OAB-q量表中症状评分均较前降低(P<0.05),应对方式等4个维度评分均较前升高(P<0.05),且灌注组改善程度优于对照组(P<0.05);两组治疗后VV、Qavg、Qmax、VUD、VFD均较前升高(P<0.05),且治疗后灌注组上述指标明显高于对照组(P<0.05)。结论:伤科黄水膀胱灌注可以提高女性膀胱过度活动症患者的临床疗效,改善症状,提高生活质量,调节尿动力学治疗。
Objective:To investigate the clinical efficacy of bladder infusion of Shangke huangshui in the treatment of overactive bladder(OAB)in female.Methods:A total of 98 female patients with OAB were randomly divided into the control group and the study group.The control group was given conventional treatment;on which basis,the study group was also treated with bladder infusion of Shangke Huangshui.After 12 weeks of treatment,clinical effectiveness was analyzed.Scores of OABSS,IUSS,PPBC,IIQ-7,UDI-6 and OAB-q were observed before and after the treatment in the two groups.VV,Qavg,Qmax,PVR VUD and VFD were measured.The numbers of urination and incontinence in 24 hours were record as well.Results:The total clinical effective rate was significantly higher in the study group than that in the control group(P<0.05).The scores of OABSS,IUSS,PPBC,IIQ-7 and UDI-6,as well as the numbers of urination and incontinence in 24 hours decreased significantly in the two groups after the treatment(P<0.05);of which the decreases were more significant in the study group(P<0.05).The symptoms scores in the OAB-q scale were lower and the scores of the four dimensions were all higher in the two groups after the treatment(P<0.05);of which the improvements were more obvious in the study group than those in the control group(P<0.05).After the treatment,the values of VV,Qavg,Qmax,VUD,and VFD were all higher than those before the treatment in the two groups(P<0.05);the changes in the study group were more significant(P<0.05).Conclusion:Bladder infusion of Shangke Huangshui can improve the clinical efficacy of OAB in female.The therapy can alleviate symptoms,improve life quality of the patients and regulate urodynamic.
