超声测量宫颈角对早产的预测价值 被引量:7

摘要 目的探讨经阴道超声测量宫颈角预测早产的应用价值。资料与方法选取304例妊娠28~32周先兆早产孕妇,其中发生早产104例为早产组,未早产200例为足月产组。所有研究对象均行经阴道超声检查以测量宫颈长度及宫颈角,比较两组经阴道超声测量宫颈长度及宫颈角对早产的预测效能。结果早产组孕妇宫颈长度为(2.06±0.25)cm,显著小于足月产组的(2.58±0.36)cm,差异有统计学意义(P<0.05);早产组孕妇宫颈角为(110.77±5.42)°,显著大于足月产组的(96.69±5.42)°,差异有统计学意义(P<0.05)。宫颈长度预测孕妇早产风险的最佳阈值为2.34 cm,曲线下面积为0.893;宫颈角预测孕妇早产风险的最佳阈值为106.15°,曲线下面积为0.948。结论与宫颈长度相比,经阴道超声测量宫颈角对预测早产具有更高的效能。当测量宫颈角≥106.15°时,应高度警惕早产的发生。 Purpose To explore the application value of transvaginal ultrasound measurement of uterocervical angle to predict preterm labor.Materials and Methods A total of 304 precursor preterm pregnant women were selected from 28 to 32 weeks of gestation.Among them,104 pregnant women who diagnosed as premature delivery was selected as the premature group,and the other 200 cases of term delivery divided into normal group.All subjects underwent transvaginal ultrasonography to measure the cervical length and uterocervical angle,and compared the two groups of transvaginal ultrasound to measure the cervical length and uterocervical angle in predicting preterm birth.Results The length of the cervix in the premature group was(2.06±0.25)cm,which was significantly smaller than the normal group of(2.58±0.36)cm(P<0.05);the cervix angle of the premature group was(110.77±5.42)°,which was significantly greater than the normal group of(96.69±5.42)°(P<0.05).The best threshold of cervical length to predict the risk of preterm birth in pregnant women was 2.34 cm,and the area under the curve was 0.893;the best threshold of cervical angle to predict the risk of preterm birth in pregnant women was 106.15°,and the area under the curve was 0.948.Conclusion Compared with the length of the cervix,the measurement of the uterocervical angle by transvaginal ultrasound is more effective in predicting preterm birth.When measuring the cervical angle≥106.15°,the occurrence of premature birth should be highly vigilant.
