术前膳食教育对老年结直肠癌术后患者营养水平、抗肿瘤免疫应答和肠道菌群的影响 被引量:2

Effect of dietary education before surgery on nutritional levels,anti-tumor immune response and intestinal flora of elderly patients with colorectal cancer

摘要 目的探讨术前基于营养风险筛查的膳食教育对老年结直肠癌术后患者营养水平、抗肿瘤免疫应答和肠道菌群的影响。方法按照随机数字表法将我院收治的老年结直肠癌行腹腔镜结直肠癌根治术治疗患者127例进行分组,对照组63例术后行常规肠内营养支持,观察组64例在对照组基础上增加术前基于营养风险筛查的膳食教育。1周后,观察两组患者营养水平、抗肿瘤免疫应答和肠道菌群的数目。结果观察组术后1周末TP、PA及TRF水平较对照组高(P<0.05);观察组术后1周末CD3+细胞、CD4+细胞及CD4+/CD8+较对照组高,CD8+细胞水平较对照组低(P<0.05);观察组术后首次排便时双歧杆菌和乳酸菌菌量较对照组多,大肠杆菌和肠球菌较对照组少(P<0.05)。结论术前基于营养风险筛查的膳食教育应用于老年结直肠癌术患者中,可通过改善营养状况,提高机体免疫功能,调节肠道菌群。 Objective To explore the effect of dietary education based on nutritional risk screening before sur⁃gery on nutritional levels,anti-tumor immune response and intestinal flora of elderly patients with colorectal cancer.Methods 127 elderly patients with colorectal cancer who underwent laparoscopic colorectal cancer radical treatment in our hospital were randomly divided into the control group and the observation group.63 cases in the control group re⁃ceived routine enteral nutritional support,while 64 cases in the observation group added dietary education based on nutri⁃tion risk screening before the operation.One week later,the nutritional level,anti-tumor immune response and number of intestinal flora of the two groups were observed.Results The levels of TP,PA and TRF in the observation group were higher than those in the control group at the end of one week after surgery(P<0.05);the CD3+cells,CD4+cells and CD4+/CD8+in the observation group were higher than those in the control group,and the level of CD8+cells was lower than that in the control group(P<0.05);the observation group had more aerobic bacteria and lactic acid bacteria during the first postoperative bowel movement than the control group,while escherichia coli and enterococci were less than the control group(P<0.05).Conclusion Dietary education based on nutritional risk screening before surgery can improve the nutritional status,improve the body's immune function and adjust the intestinal flora.
