摘要
目的分析托里消毒饮联合象皮生肌膏治疗慢性皮肤溃疡的临床效果。方法选取2015年6月-2018年12月于湖南中医药大学第一附属医院治疗的慢性皮肤溃疡患者108例,采用随机数字表法将其分为对照组和治疗组,各54例。两组均使用象皮生肌膏治疗,治疗组则加用托里消毒饮。观察并比较两组临床疗效、细胞因子指标[白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和血管内皮生长因子(VEGF)]、溃疡愈合指标和不良反应情况。结果治疗组总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗后,两组IL-6、TNF-α水平均低于治疗前,VEGF水平高于治疗前,且治疗组IL-6、TNF-α水平均低于对照组,VEGF水平高于对照组,差异有统计学意义(P<0.05)。治疗组愈合时间、生肌时间和腐肉净化时间明显短于对照组,差异有统计学意义(P<0.05)。两组均未出现明显不良反应。结论托里消毒饮联合象皮生肌膏治疗慢性皮肤溃疡的效果显著,可促进创面更快愈合,降低炎症因子水平,安全可靠。
Objective To analyze clinical effect of Tuoli Xiaodu Decoction combined with Xiangpi Shengji Ointment in the treatment of chronic skin ulcer.Methods One hundred and eight patients with chronic skin ulcer treated in the First Affiliated Hospital of Hu′nan University of Chinese Medicine from June 2015 to December 2018 were selected and divided into control group and treatment group by random number table method,with 54 cases in each group.Both groups were treated with Xiangpi Shengji Ointment,while treatment group was treated with Tuoli Xiaodu Decoction.Clinical efficacy,cytokine indexes(interleukin 6[IL-6],tumor necrosis factor-α[TNF-α],vascular endothelial growth factor[VEGF]),ulcer healing indexes and adverse reactions of two groups were observed and compared.Results Total effective rate in treatment group was significantly higher than that in control group(P<0.05).After treatment,the levels of IL-6 and TNF-αin two groups were lower than those before treatment,and the level of VEGF in two groups was higher than that before treatment,the levels of IL-6 and TNF-αin treatment group were lower than those in control group,and VEGF level in treatment group was higher than that in control group,the differences were statistically significant(P<0.05).The healing time,myogenic time and putrid purification time in treatment group were significantly shorter than those in control group,the differences were statistically significant(P<0.05).There were no obvious adverse reactions in both groups.Conclusion Tuoli Xiaodu Decoction combined with Xiangpi Shengji Ointment has significant effect in treating chronic skin ulcer,which can promote wound healing faster,reduce the level of inflammatory factors,and it is safe and reliable.
