摘要
目的探讨磁敏感加权成像中磁敏感血管征(SWI-SVS)与MRA在显示大脑动脉闭塞中应用。方法回顾性连续纳入130例发病6h内前循环脑梗死急性期患者,均接受头部CT、MRI检查,并记录患者基线情况。对CT及MRI进行分析,记录SVS存在的情况,共有33例SWI-SVS与DSA或CTA结果对照一致。结果以DSA或CTA为参照标准,SVS、MRA对急性MCA主干闭塞的阳性率分别为54.5%(18/33),84.8%(28/33),其中SVS显示大脑中动脉远端血栓优于MRA。结论MRA准确度高于SVS,MRA在大脑动脉闭塞中具有良好的适用性,同时SVS在显示大脑中动脉远端分支血栓具有优势,MRA在显示颈内动脉闭塞方面具有优势,因此SVS可对MRA起到辅助诊断的作用,两者具有互补作用,两者都可为急性缺血性卒中的早期诊断、治疗方式的选择及溶栓后血管再通预测提供参考依据。
Objective To discuss the application of susceptibility vessel sign and MRA in evaluation of cerebral artery occlusion.Methods A total of 130 patients with acute anterior circulation infarction within 6 h onset were retrospectively reviewed.All patients underwent one-stop CT and MRI examinations,and the baseline condition was recorded.Results With DSA or CTA as the reference standard,the positive rate of SVS and MRA for acute MCA trunk occlusion was 54.5%(18/33)and 84.8%(28/33),and SVS showed that the thrombus of the distal brain was superior to MRA.Conclusion The accuracy of MRA is higher than that of SVS,MRA has good applicability in cerebral artery occlusion,and SVS has advantages in displaying distal branches of middle cerebral artery.MRA has the advantage in displaying internal carotid artery occlusion,so SVS can play an auxiliary role in the diagnosis of MRA.Both can provide reference for the early diagnosis of acute ischemic stroke,the choice of treatment mode and the prediction of vascular recanalization after thrombolysis.
