摘要
目的为解决医院疾病诊断相关分组(Diagnosis Related Groups,DRGs)付费既满足治疗需求又不超付费标准的难题,设计基于医院信息系统临床路径的DRGs费用管控系统,在规范诊疗的同时有效降低医疗成本。方法采用自动匹配疾病DRGs、路径费用模板和路径外医嘱监控的方法,实现事前费用测算、事中费用对比提醒、事后费用统计分析。结果该系统能够在正常的诊疗过程中进行DRGs费用有效管理。试点科室4个临床路径病种系统使用前后的平均住院费经t检验比较差异均有统计学意义,使用后平均住院费下降了5.8%。结论该系统实现了医院资源的优化管理,利用有效的资源,使患者得到更好的治疗效果。
Objective In order to solve the problem that diagnosis related groups(DRGs)payment not only meets the treatment needs but does not exceed the payment standard,based on the clinical path of the hospital information system,the DRGs cost management and control system is designed to standardize diagnosis and treatment while effectively reducing medical costs.Methods The method of automatically matching DRGs,path cost templates,and off-path doctor’s order monitoring were adopted to realize preexpense calculation,in-process cost comparison reminder,and post-expense statistical analysis.Results The system could effectively control the cost of DRGs in the course of normal diagnosis and treatment.T test was used to compare the difference of the average hospitalization expenses between before and after the use of the system in the four clinical pathways.After useing the average hospitalization expenses decreased by 5.8%.Conclusion The system is characteristic with optimized management of hospital resources and effective use of resources to make patients get better results.
