摘要
【目的】观察关节错缝术配合针刺疗法治疗老年腰椎间盘突出症的临床疗效。【方法】将120例老年腰椎间盘突出症患者随机分为关节错缝术组、针刺组和关节错缝术配合针刺组,每组各40例,关节错缝术组给予腰椎斜扳法与整盆法治疗,针刺组给予针刺治疗,关节错缝术配合针刺组给予关节错缝术配合针刺治疗。3组患者均每2 d治疗1次,治疗7次为1个疗程,共治疗4个疗程。治疗8周后,观察3组患者治疗前后日本骨科协会评估治疗分数(JOA)评分的变化情况,并评价3组患者的临床疗效。【结果】(1)治疗后,3组患者的JOA评分均明显改善(P<0.01),且关节错缝术配合针刺组改善最明显,关节错缝术配合针刺组JOA评分明显优于关节错缝术组和针刺组,差异均有统计学意义(P<0.01)。(2)关节错缝术组总有效率为80.0%(32/40),针刺组为77.5%(31/40),关节错缝术配合针刺组为95.0%(37/40)。关节错缝术配合针刺组疗效明显优于关节错缝术组和针刺组,差异有统计学意义(P<0.05)。【结论】关节错缝术配合针刺治疗老年腰椎间盘突出症,能明显改善患者的腰椎功能,临床疗效显著。
Objective To observe the clinical efficacy of manipulation for reduction of joint dislocation combined with acupuncture in treating elderly patients with lumbar disc herniation. Methods A total of 120 elderly patients with lumbar disc herniation were randomly divided into manipulation for reduction of joint dislocation group,acupuncture group, and manipulation for reduction of joint dislocation combined with acupuncture group, 40 cases in each group. The manipulation for reduction of joint dislocation group received lumbar vertebra oblique traction and reduction of pelvis,the acupuncture group received acupuncture,and the manipulation for reduction of joint dislocation combined with acupuncture group received manipulation for reduction of joint dislocation combined with acupuncture treatment. One time every two days,7 times constituted one treatment course,and the treatment covered 4 courses. After 8 weeks of treatment,the changes in Japanese Orthopaedic Association Scores(JOA)of the 3 groups before and after treatment were observed,and the clinical efficacy was evaluated. Results(1)After treatment,the JOA in the 3 groups was significantly improved(P < 0.01),and the improvement in JOA of the manipulation for reduction of joint dislocation combined with acupuncture group was significantly superior to that of the manipulation for reduction of joint dislocation group and acupuncture group(P < 0.01).(2)The total effective rate was 80.0%(32/40)in the manipulation for reduction of joint dislocation group,77.5%(31/40)in the acupuncture group, and 95.0%(37/40)in the manipulation for reduction of joint dislocation combined with acupuncture group. The therapeutic effect in the manipulation for reduction of joint dislocation combined with acupuncture group was superior to that in the manipulation for reduction of joint dislocation group and acupuncture group(P < 0.05).
