摘要
目的调查分析精神分裂症患者暴力攻击行为与精神残疾程度的相关性及影响因素。方法选取2015年6月至2018年6月期间在湖州市某三甲专科医院就诊的精神分裂症患者105例,将暴力行为攻击量表(Modified Overt Aggression Scale,MOAS)评分≥4分的患者分入观察组,评分<4分的患者分入对照组。采用简明精神病量表(Brief Psychiatric Rating Scale,BPRS)、世界卫生组织残疾评定量表Ⅱ(WHO Disability Assessment ScheduleⅡ,WHO-DASⅡ)及修订版外显攻击行为量表(Modified Overt Aggression Scale,MOAS)对105例新入院精神分裂症患者进行评估。结果观察组的言语攻击、财产攻击、自我攻击、体力攻击评分及MOAS总分高于对照组,差异均有统计学意义(p<0.05)。MOAS总分与精神残疾程度呈正相关。多因素分析结果显示,性别、入院次数、入院形式、病程、既往攻击史、精神残疾程度及疾病严重性是新入院精神分裂症患者暴力攻击的影响因素。结论精神分裂症患者的暴力攻击行为与精神残疾程度呈正相关,且精神残疾程度较重的患者更容易发生攻击行为。可通过对新入院精神分裂症患者精神残疾程度评估,防控其攻击行为发生。
Objective To investigate the correlation between violent aggression and mental disability level and influencing factors of violent aggression in patients with schizophrenia.Methods A total of 105 patients with schizophrenia who visited a tertiary specialist hospital in Huzhou City from June 2015 to June 2018 were selected.The patients with Modified Overt Aggression Scale score(MOAS)≥4 were divided into the observation group,and MOAS score<4 were divided into the control group.The 105 schizophrenia patients who were newly admitted were evaluated using the Brief Psychiatric Rating Scale(BPRS),WHO Disability Assessment ScheduleⅡ(WHO-DASⅡ),and Modified Overt Aggression Scale(MOAS).Results The scores of verbal attack,property attack,self-attack,physical attack,and MOAS total score,were all higher in the experimental group than in the control group,and the differences were statistically significant(p<0.05).The MOAS total score is positively correlated with the degree of mental disability.Multivariate analysis showed that gender,number of admissions,way of admission,course of disease,admission frequency,past attack history,mental disability degree and disease severity were the influencing factors of violent aggressions in newly admitted patients with schizophrenia.Conclusion The violent aggression is positively correlated with the degree of mental disability in newly admitted schizophrenic patients,and patients with severe mental disability are more likely to have violent aggression.The evaluation of mental disability degree of newly admitted schizophrenic patients can help prevent violent aggressions.
