摘要
复方口服避孕药(COC)的应用历史已超过50余年,经过对其成分及剂量的不断改进,扩大了临床适应范围。COC根据成分中雌激素和孕激素的不同药理作用,使其在妇产科临床工作中不仅应用于避孕,还可用于流产术后、子宫内膜异位症、功能失调性子宫出血、月经过多、调经、多囊卵巢综合征、原发性痛经、经前期综合征、女性痤疮等的治疗及某些肿瘤的预防。在实际临床应用中因所给COC的种类、剂量、给药时间等因素的不同,通过对下丘脑-垂体-卵巢轴正负反馈的调节而达到不同的治疗目的。本文针对COC在临床工作中的应用范围及作用机理进行综述,以期为COC的临床应用提供借鉴经验。
The application of compound oral contraceptive(COC)has a history of over fifty years.Through persistent improvement on composition and dosage,COC's application range has been continuously enlarged in clinical practice.According to different pharmacological mechanisms of estrogen and progesterone in the composition,COC can not only be applied to contraception,but also to the treatment of surgical abortion postoperative recovery,endometriosis,dysfunctional uterine bleeding,menometrorrhagia,menstruation regulating,polycystic ovarian syndrome,primary dysmenorrhea,premenstrual syndrome,female acne,etc and the prevention of certain tumor.In clinical practice,due to different type,dosage,medicine taking-time of COC and other factors,positive and negative feedback of hypothalamic-pituitaryovarian axis can be regulated so that various treatment goals can be achieved.The paper aims to summarize the clinical application range and work mechanism of COC,in order to provide reference experience for the clinical application of COC.
