摘要
目的探讨小汗腺汗孔瘤的病理诊断和鉴别诊断。方法选取清远市人民医院2011年至2019年收治的经病理确诊的8例小汗腺汗孔瘤患者,回顾性分析其临床资料、组织病理学特点和预后。结果8例患者中,发生于足底4例,发生于头皮、肩部、小腿及眉弓各1例。皮损表现为红色、淡红色或褐色的丘疹或疣状赘生物,高出皮肤表面。镜下见肿瘤组织局部与表皮基底部相连,并替代表皮细胞向真皮层广泛增生,且与相邻表皮的角质形成细胞分界清晰,部分形成网状结构,瘤细胞形态单一、小立方状,细胞核圆形或椭圆形、嗜碱,可见核分裂象;肿瘤细胞间见纤维血管间质。8例患者均行手术切除,随访1.5个月~8年,均无复发迹象。结论小汗腺汗孔瘤易误诊为其他皮肤疾病,如化脓性肉芽肿、血管瘤、疣、胼胝等,确诊需依靠组织学活检。
Objective To investigate the pathological diagnosis and differential diagnosis of eccrine poroma.Methods Eight cases of eccrine poroma diagnosed by pathology from 2011 to 2019 in Qingyuan People’s Hospital were selected. And their clinical data, histopathological features and prognoses were analyzed retrospectively.Results Of the 8 cases, 4 cases occurred in the sole of the foot, 1 case in the scalp, 1 case in the shoulder, 1 case in the lower leg and 1 case in the eyebrow arch. Skin lesions appeared as red, reddish, brown papules or verrucous vegetations, higher than the skin surface. Microscopically, the tumor tissue was connected with the basal part of epidermis, and proliferated extensively to the dermis in place of epidermal cells. Moreover, it had a clear boundary with the keratinocytes of the adjacent epidermis. Some tumor tissues formed a reticular structure. The shape of tumor cells was single, small cubic, with round, oval and basophilic nuclei. Mitotic figures could be seen among the nuclei. Fibrous vascular stroma was seen between the tumor nests. All the 8 cases underwent surgical resection. There were no signs of recurrence after 1.5 months to 8 years of follow-up.Conclusions Eccrine poroma is easy to be misdiagnosed as other skin diseases, such as pyogenic granuloma, hemangioma, verruca, corpus callosum and so on. Definite diagnosis of eccrine poroma depends on biopsy.
