摘要
目的:观察关节松动术结合冲击波治疗僵硬期冻结肩的临床疗效。方法:将符合纳入标准的80例患者随机分成关节松动术结合冲击波组(治疗组)40例和关节松动术组(对照组)40例。对照组采用澳大利亚Maitland手法,治疗组在此基础上加入冲击波治疗,均治疗2疗程。以《中医病证诊断与疗效标准》中的肩周炎疗效标准及Constant-Murley肩关节评分来比较疗效。结果:治疗组总有效率为92.5%;对照组总有效率为75%,2组比较差异有统计学意义(P<0.05);Constant-Murley肩关节评分表明治疗前后各组内比较,差异均有统计学意义(P<0.05),组间治疗后分数差异有统计学意义(P<0.05),说明治疗组在减轻患者疼痛,增加关节活动度及肌力,提高总分方面均优于对照组。结论:关节松动术结合冲击波能更有效地治疗僵硬期冻结肩。
Objective:To observe the clinical effect of joint mobilization combined with shock wave in the treatment of frozen shoulder in stiff stage.Methods:Eighty patients who met the inclusion criteria were randomly divided into two groups:the joint mobilization combined with shock wave group(study group,40 cases)and the joint mobilization group(control group,40 cases).The control group was treated with Australian Maitland mobilization,and the study group was added with shock wave therapy on this basis.They were both treated for 2 courses.The curative effect standard of shoulder periarthritis in TCM syndrome diagnosis and curative effect standard,as well as Constant-Murley shoulder joint score were used to compare the efficacy.Results:The total effective rate in the study group was 92.5%while the control group was 75%(P<0.05);Constant-Murley shoulder score showed that there were statistically significant differences in each group before and after treatment(P<0.05).There were statistically significant differences in scores between groups after treatment(P<0.05)as well.The results showed that the study group was superior to the control group in alleviating pain,increasing the joint motion and muscle strength,and improving the total score of the patients.Conclusion:Joint mobilization combined with shock wave can be more effective in the treatment of frozen shoulder in stiff stage.
