摘要
目的探究稳定期慢性阻塞性肺疾病患者小气道参数与肺功能指标间的相关性。方法收集2018.12月至2020.01月在阜阳市人民医院随诊的稳定期慢阻肺患者58例为研究组,分为轻-中度肺功能组(30例)及重-极重度肺功能组(28例),另收集同期健康体检者37例为对照组,比较三组小气道参数有无统计学差异;分析研究组小气道参数及肺功能指标间相关性,并将研究组根据影像分为支气管管壁增厚组(30例)及管壁非增厚组(28例),比较两组肺功能指标有无统计学差异。结果研究组小气道壁厚、肺气肿占比、壁厚与气管腔直径比值大于对照组,重-极重度肺功能组中,气管腔直径、气管腔面积小于轻-中度肺功能组及对照组,有统计学差异(P<0.05);研究组中各项肺功能指标与小气道参数间存在相关性(P<0.05);支气管管壁增厚组中1秒率、1秒量占预计值的百分比、最大呼气中期流速占预计值的百分比低于管壁非增厚组,具有统计学差异(P<0.05)。结论CT小气道参数与肺功能指标具有相关性,可反应气流受限严重程度,对慢阻肺的诊治具有一定价值。
Objective To explore the correlation between the parameters of small airway and pulmonary function in patients with stable chronic obstructive pulmonary disease.Methods 58 stable COPD patients who were followed up in Fuyang People's Hospital from December 2018 to January 2020 were collected as the study group,which was divided into the light medium lung function group(30 cases)and the heavy extremely severe lung function group(28 cases).37 healthy people during the same period were selected as the control group,and the differences of small airway parameters among the three groups were compared.The correlation between small airway parameters and lung function indexes of the study group was analyzed,and the study group was divided into two groups according to their images:the thickening group(30 cases)and the non thickening group(28 cases).The differences of pulmonary function between the two groups were compared.Results WT,Laa%,and T/D in the study group were higher than those in the control group,LD and AI in the severe pulmonary function group were lower than those in the mild-moderate pulmonary function group and the control group(P<0.05),and there was a correlation between various pulmonary function indexes and small airway parameters in the study group(P<0.05).The FEV 1/FVC,FEV 1%,and MMEF75/25%in the bronchial wall thickening group were lower than those in the non-thickening group(P<0.05).Conclusion The parameters of small airway are correlated with the indexes of pulmonary function,which can reflect the severity of airflow limitation and have certain value in diagnosis and treatment of COPD.
